Toplum temelli ruh sağlığı hizmet modelinin çekirdeğini oluşturan toplum ruh sağlığı merkezlerindeki amaç, belli bir coğrafi alanda yaşayan ağır ruhsal hastalığı olan hastaları merkezde kayıt altına almak, gezici ekiplerle düzenli takip, tedavi ve rehabilitasyonlarını sağlayarak topluma yeniden kazandırmaktır. Eskiden sadece hastalandıkları zaman, hastanelere dayalı kurumsal tabanlı tedavi modelinden faydalanan hastalar taburculuk sonrası takiplerini aksatmakta ve kısa süre sonra tekrarlayan hastalıkları nedeni ile hastaneye yatışları sıklıkla yapılmaktaydı. Oysa bugün toplum ruh sağlığı merkezleri ile amaç hastanelere dayalı kurumsal tabanlı hizmet modelinden, toplum tabanlı ruh sağlığı hizmet modeline geçmektir. Bu modelde hastaların kontrole gelişi beklenmeden gezici ekiplerle ev ziyaretlerinin yapılması; hastaların kendi mahallinde düzenli takip ve tedavilerinin gerçekleştirilerek iyilik hallerinin korunması, atakların önlenerek; hastane yatışlarının azaltılması ve rehabilitasyonlarının sağlanmasıyla hastaların topluma yeniden kazandırılması hedeflenmektedir. Bu yazıda Türkiye'de ilk kez Mayıs 2008'de açılan Bolu Toplum Ruh Sağlığı Merkezi'nde ki uygulamalardan söz edilecektir.

Community mental health centers are the main part of community based psychiatry model in which the target depends on following frequently the patients with severe mental illness who are living in a catchment area, arrenging their medicine and rehabilitation in order to adapt them back to community. Before the community based psychiatry model, the patients were following by the system based on hospitals in which they give up their medicine after a short time from discharge and then rehospitalized frequently. However, the purpose of the community mental health centers is to change the system from hospital based psychiatry model to community based psychiatry model. The target of this model is to follow the treatment of the patients with severe mental illness frequently in the community by the mobile team's home visits and rehabilitation in order to prevent the attacks and rehospitalization and readapt them to community. In this article the practices of the first community mental health center of Turkey which is opened in Bolu on 2008, May will be explained.
