Karışık Madde Kullanımı Olan Bir Ası Olgusu

Ası, boynun bir bağ ile sıkılmasıdır. Bağın vücut ağırlığının indirekt etkisi ile sıkılması sonucu oluşur. Ası için, sıklıkla görülmekle birlikte her zaman tamamen dik bir pozisyonda bulunulması ve ayakların yerden ayrılması gerekmez. Ası, hükmi olanlar dışında çoğunlukla intihardır. Bazı olgularda ise kaza sonucu olur.
Bu çalışmada İstanbul Adli Tıp Kurumunda otopsisi yapılan ve karışık madde kullanımı olan ası sonucu ölüm olgusu değerlendirilmiştir. Olgunun otopsi verileri ve kimyasal analiz sonuçları değerlendirilerek, yaş, cinsiyet, olay yeri, ası tipi ve ası vasıtası, hyoid kemik, tiroid kartilaj kırığı olup olmadığı, kan ve idrardaki alkol ve diğer maddelerin düzeyi açısından incelenerek literatür verileri ile tartışılması amaçlanmıştır.
Evinde ası sonucu ölü olarak bulunan, 26 yaşındaki erkek olgunun thyroid kartilajında kırık ile telem altına uyan boyun yumuşak dokusunda ekimoz saptanmıştır. Kimyasal analizde kan ve idrarda etanol, morfin metaboliti, benzodiazepin türevi ve esrar metaboliti bulunmuştur. Otopsi ve otopsi sonrası yapılacak toksikolojik, histopatolojik incelemeler her ası olgusunda ayırıcı tanı ve olayın ası ile intihar ya da öldükten sonra ası olayı olup olmadığının tespiti açısından önemlidir.
Hanging Who Used Mixture Substance: A Case Report
Hanging is a constriction of the neck by a ligature, constricting force being applied indirectly to the ligature through the weight of the body. Hanging need not to be in the fully erect posture, with the feet clear of the ground, though this often happens. Excluding judicial execution, hanging is mostly suicidal, though some cases are accidental.
In this study, a case that performed autopsy in State Institute Forensic Medicine Istanbul because of hanging who used mixture substance was presented. For this aim, autopsy report of the case was investigated. The age, gender, crime scene, type of hanging and noose, injuries to the hyoid bone and the thyroid cartilage, level of blood alcohol and other drugs, material that used, were reviewed.
The age of the victim was 26. The crime scene was the house of the victim. Thyroid cartilage fracture and bruises in the soft tissue of the neck were reported. Ethanol, opium, benzodiazepine and cannabinoid were detected in the blood and urine. In every hanging case, autopsy, histopathological and toxicological examinations are important for distinguishing between suicidal hanging and the suspension of a body after death.
