Şizofreni Olgularında Tedaviye Uyum: Üç Yıllık Doğal İzlemede Olanzapin, Risperidon ve Klasik Antipsikotiklerin Karşılaştırılması

Amaç: İlaç uyumu şizofreninin gidişini belirleyen önemli bir etkendir. Ancak, olguların önemli bir bölümünde
tedaviye uyum yeterli değildir. Bu çalışmada olanzapin, risperidon ve klasik antipsikotik kullanan
olguların üç yıllık doğal süreçte tedaviye uyumu ve uyum ile ilişkili etkenlerin saptanması amaçlanmıştır.
Yöntem: Çalışma Şizofreni ve Diğer Psikotik Bozukluklar Ayaktan İzleme Ünitesinde gerçekleştirilmiştir. Antipsikotik
seçimi klinik ölçütlere göre yapılmıştır. Üç yıllık doğal izlemeye olanzapin (n=28), risperidon
(n=45) ve klasik antipsikotik (n=31) kullanan toplam 104 olgu alınmıştır. Hasta ve/veya yakınlarının bildirimine
göre ilaç uyumsuzluğuna neden olan etkenler saptanmıştır. Belirlenen zaman aralığının %20 veya
daha fazlasında ilaç kullanımını aksatmak uyumsuzluk ölçütü olarak alınmıştır.
Sonuç ve Yorum: İzleme döneminde uyumsuzluk oranının %40.4 olduğu saptanmıştır. Uyumsuzluk antipsikotik
gruplarında benzer bulunmuştur (olanzapin: 25%, risperidon: 46.7%, klasik antipsikotikler 45.2%).
Hastalar tarafından uyumsuzluğun başlıca nedenleri yetersiz tedavi yararı (%7.7), ilaç yan etkileri (27.9)
olarak bildirilmiştir. Sonuç olarak, üç yıllık doğal izlemede ikinci kuşak antipsikotiklerin iyi tedavi uyumunda
klasik antipsikotiklerden farklı olmadığı saptanmıştır.
Medication compliance in schizophrenia. A 3- [year] naturalistic follow-up study comparing olanzapine, risperidone and conventional antipsychotics
Objective: Medication compliance is an important determinant in schizophrenia treatment outcomes. However, many patients with schizophrenia
are poorly adherent with antipsychotic medications. Second-generation antipsychotics may be more acceptable to patients and result in
increased adherence. This study aimed to investigate medication compliance in cases of patients receiving a single antipsychotic medication
(conventional antipsychotic, olanzapine, risperidone) among outpatients with schizophrenia, and examine whether their antipsychotic therapy
and other factors may affect the medication compliance.
Methods: The study was carried out at the schizophrenia outpatient unit of the Department of Psychiatry, Gulhane School of Medicine, Ankara.
Outpatients with schizophrenia entered this prospective, naturalistic study when they received a new prescription for an antipsychotic drug.
Treatment assignment was based on purely clinical criteria, as the study did not include any experimental intervention. The selected subjects
were surveyed for information including their medication compliance, sociodemographics, treatment-related side effects, and perceived
treatment related benefits. Of the 104 subjects surveyed, 73 received second-generation antipsychotics (risperidone or olanzapine), while
31 received conventional antipsychotics (haloperidol or zuclopenthixol). Patients were interviewed at regular intervals over a 3- [year] period.
Results and Conclusion: Noncompliance was 40.4% over three [year]s. There were no significant differences between olanzapine (25%),
risperidone (46.7%) and conventional agents (45.2%) with respect to rates of noncompliance. Poor medication compliance was related to
perceived treatment benefits, antipsychotic side effects, and other factors. Second-generation antipsychotics had no correlation with good
medication compliance.
