Capgras sendromu (CS) genellikle bir yakınının fiziksel olarak çok benzeyen biriyle değiştiğine sanrısal inancın
olmasıyla belirlidir. Nadir olarak görülen CS, sıklıkla şizofreninin paranoid alttipinin seyri sırasında, daha
seyrek olarak şizoafektif ve duygudurum bozuklukları ile birlikte görülür. Önceleri CS’nun psikodinamik
çatışmalardan kaynaklandığı düşünülmüş olmakla birlikte, son yıllarda organik etiyolojiye bağlı yayımlanmış
olgu sayısında belirgin bir artış görülmektedir. Bu yazıda, klozapin kullanan bir şizofreni hastasında aşırı
kilo alımından dolayı ilacın kesilmesi sonucu ortaya çıkan, kullanılan diğer antipsikotik ilaçlara yanıt vermeyen
ve diğer psikotik belirtilerinde de artış görülen ve tekrar klozapin başlanmasına rağmen geçmeyen bir CS
olgusunu bildiriyoruz. Literatürde buna benzer herhangi bir bildiri mevcut değil ve bu durumu açıklayabilecek
yeterli bilgiye de sahip değiliz.

Capgras syndrome (CS) is characterized by the delusional belief that a person, usually very close to the patient, has been replaced by a double
who is physically very similar to the original. CS is relatively rare, occurring predominantly in course of schizophrenia, particularly of the paranoid
sub-type, and less frequently in association with schizoaffective and affective disorders. Recent [year]s have witnessed a sharp increase in the
number of published CS cases with an organic etiology, however, CS was considered to have its origins in psychodynamic conflict. We present
a patient with CS which appeared after cessation of clozapine due to excessive weight gain in a patient with preexisting schizophrenia.
CS did not respond to any other antipsychotic and symptoms of CS had not resolved after reapplication of clozapine. This is a unique case
which does not resemble any other in the literature and we have little information to explain this situation.
