Kronik Pelvik Ağrı Sendromunda Psikiyatrik Değerlendirme

Amaç: Kronik pelvik ağrı sendromu (KPAS), pelvik ağrının ön planda olduğu bir klinik tablodur. Emosyonel
sorunlar etyolojik bir faktör olarak belirtilmekte ancak net tanım yapılamamaktadır. Araştırmamızda, KPAS
hastalarını psikiyatrik açıdan değerlendirmek ve eşlik eden psikiyatrik semptomları belirlemek amaçlanmıştır.
Yöntem: Gaziosmanpaşa Tıp Fakültesi Üroloji Polikliniği’nde izlenen 40 KPAS tanılı erkek hasta ile benzer
sosyodemografik özelliklerde 35 erkek kontrol araştırmaya katıldı. Her iki gruba yapılandırılmış psikiyatrik
görüşme (SCID-I), Hastane Anksiyete Depresyon Ölçeği (HADÖ), Belirti Tarama Listesi (SCL-90-R) uygulandı,
hasta grubunun ağrısı Vizüel Analog Skala (VAS) ile ölçüldü.
Sonuç: KPAS’lı grup kontrol grubuna oranla anlamlı düzeyde daha fazla psikiyatrik tanı almıştır (p=0.01).
SCL-90-R ile ölçülen belirtilerde KPAS hasta grubunda somatizasyon, obsesyon kompulsiyon, depresyon,
anksiyete ve PSDI puanları anlamlı düzeyde daha yüksek bulunmuştur (p<0.05). KPAS’lı grupta ağrının
şiddeti ile SCL-90-R’nin somatizasyon, obsesyon kompulsiyon, anksiyete, öfke, fobik anksiyete alt ölçekleri,
Genel Belirti Düzeyi ve Pozitif Belirti Düzeyi ile HADÖ’nün anksiyete alt ölçek puanlarının da arttığı
belirlenmiştir.
Yorum: Sonuçlarımız, KPAS hastalarının psikiyatrik belirtiler açısından dikkatle değerlendirilmesi gerektiğini
göstermektedir.
Psychiatric evaluation in chronic pelvic pain syndrome
Objective: Chronic prostatitis (CP) is a syndrome in males characterized by pelvic pain with or without voiding symptoms. The majority of
men with symptomatic prostatitis are category III according to the National Institutes of Health classification scheme, or CP/chronic pelvic pain
syndrome (CP/CPPS). Its causes are poorly understood and evidence-based treatments are lacking. Therapuetic attempts have frequently
produced unsatisfactory results. Although psychological distress has been proposed as an etiological factor, the literature lacks prospective
studies using standardized measures. Psychosomatic factors have often been neglected in CPPS.
Method: This study examined men with CPPS (n=40) and a control group of men (n=35) equivalent on demographic characteristics, and
all were compared on pschological measures. Both groups were applied the Structured Clinical Interview for DSM-IV Axis Disorders (SCID-I),
the Hospital Anxiety and Depression Scale (HADS), the Symptom Check List 90-R (SCL-90-R), and pain severities were measured by the Visual
Analogue Scale (VAS).
Conclusion: CPPS patients had significantly higher scores on somatization, obsession compulsion, depression, anxiety, and PSDI subscale of
the SCL-90-R, compared to controls (p=0.01). Besides, the CPPS patient group had significantly higher frequency of a psychiatric disorder. In
the CPPS group, perceived severity of pain (VAS scores) showed significant positive correlations with somatization, obsession compulsion,
anxiety, öfke, phobic anxiety subscales and Global Symptom Index and Positive Symptom Distress Index of the SCL-90-R and anxiety subscale
of the HADS. The results indicate that emotional distress is common among CPPS patients, which shows the need for careful evaluation and
attention to psychological symptoms.
