Şizofreni ve Bipolar Bozukluklu Hastalarda Tipik ve Yeni Nesil Antipsikotiklerin Hematolojik Parametreler Üzerine Etkileri

Amaç: Çalışmada, şizofreni ve bipolar bozukluklu hastalarda tipik ve yeni nesil antipsikotiklerle tedavinin
hematolojik parametreler üzerine etkilerinin araştırılması planlanmıştır.
Yöntem: Çalışmaya DSM-IV tanı ölçütlerine göre şizofreni veya bipolar bozukluk tanısı konulan 66 hasta ve
21 sağlıklı kontrol dâhil edilmiştir. Hastalar terapötik dozlarda 5 hafta antipsikotik tedavi almışlardır. Tedavi
öncesinde ve sonrasında hematolojik parametrelerden beyaz hücre sayısı, nötrofil ve lenfosit oranları,
hemoglobin, hematokrit, ortalama eritrosit hacmi, ortalama eritrosit hemoglobini değerleri ölçülmüştür.
Bulgular: Hastalarda tedavi öncesinde nötrofil oranı kontrollerinkinden yüksek, lenfosit oranı ise düşük
bulunmuştur. Antipsikotik tedavi ile her ikisinin oranları da düşmüştür. Yeni nesil antipsikotikler hematolojik
parametreleri pek fazla etkilemezken, tipik antipsikotikler nötrofil oranlarını tedavi öncesine göre düşürmüş,
lenfosit oranlarını da tedaviden sonra kontrollerinkinden düşük hale getirmiştir.
Sonuç: Özellikle nötrofil ve lenfosit oranları gibi hematolojik parametrelerde hem hastalığın etkisi hem
antipsikotik tedavinin etkisi ile değişiklikler olmaktadır. Antipsikotiklerin nötropeni ve lenfopeni yapıcı etkisi
özellikle tipik antipsikotiklerden kaynaklanıyor gibi görünmektedir.
The Effects of Typical and New-Generation Antipsychotics on Hematological Parameters in Patients with Schizophrenia and Bipolar Disorder
Objective: The study aimed to investigate the effects of new-generation and typical antipschotic treatment on hematological parameters in
schizophrenic and bipolar patients.
Method: Sixty-six schizophrenic and bipolar inpatients diagnosed according to DSM-IV criteria and 21 healthy subjects were recruited for the
study. The patients received antipsychotic treatment at therapeutic doses for 5 weeks. Hematological parameters such as total white blood
cell count, neutrophil and lymphocyte percentages, hemoglobin, hematocrit, mean corpuscular volume, and mean corpuscular hemoglobin
were measured before and after the treatment in the patients.
Results: The patients had significantly higher neutrophil and lower lymphocyte percentage than the controls. Antipsychotic treatment decreased
both percentages of neutrophil and lymphocyte. Although new-generation antipsychotics had no effect on hematological parameters,
typical antipsychotics decreased neutrophil percentage in post-treatment compared to that in pre-treatment in the patients and decreased
lymphocyte percentage in post-treatment in the patients compared to that of the controls.
Conclusion: Hematological parameters such as neutrophil and lymphocyte percentages are affected by both the desease and antipsychotic
treatment. Neutropenic and lymphopenic effect of antipsychotics seems pertaining to typical antipsychotics.
