Dirençli iki uçlu bozukluğu ve konjenital hipotiroidisi olan hastada tiroksin güçlendirme tedavisi: Olgu sunumu

Dirençli iki uçlu bozuklukta tiroksin güçlendirme tedavisinin etkinliği ve döngü sıklığını azalttığı bilinmektedir. Bu
yazıda, konjenital hipotiroidi nedeniyle tiroksin yerine koyma tedavisi alırken ötiroid durumda iki uçlu bozukluk
manik nöbet geçiren bir olgunun tedavisindeki güçlük ve buna bulunan çözüm tartışılmıştır. Bu hastada elektrokonvulzif
tedavi (EKT), tipik ve atipik antipsikotik tedaviler, etkin doz ve sürede uygulanmasına karşın yanıt alınmaması
üzerine tedaviyi güçlendirmek için almakta olduğu tiroksin dozu fizyolojik düzeylerin üzerinde artırılmıştır.
Konjenital hipotiroidide ötiroid durumda, yerine koyma tedavisi için önerilenin üzerinde dozlarda tiroksin uygulamasının
iyi tolere edildiği ve tedaviye yanıt alındığı gözlenmiştir. Bu yazıda daha önceden de bilinen tiroksin
güçlendirme tedavisinin dirençli iki uçlu bozuklukta fizyolojik sınırların üzerinde de kullanılabileceğinin vurgulanması
amaçlanmıştır. Ayrıca literatürde ilk kez zaten konjenital hipotiroidisi olan bir hastada ortaya çıkan dirençli bir
iki uçlu bozukluğa müdahale anlatılmaktadır.
Thyroxine augmentation therapy in a patient with resistant bipolar disorder and congenital hypothyroidism: case report
The efficacy of thyroxine augmentation therapy in resistant bipolar disorder, is known to reduce the cycle
frequency and be efficient. In the present report, the difficulties and solutions of treating a patient displaying
bipolar disorder manic attack in an euthyroid state while at the same time receiving thyroxine replacement therapy
for congenital hypothyroidism, were discussed. Because no response could have been gained as a result of the
Electro Convulsive Therapy (ECT) and typical/atypical antipsychotic treatments applied in appropriate dose and
durations, thyroxine augmentation with supraphysiological levels, was added to the general treatment scheme. In
euthyroiditis condition of congenital hypothyroidism, thyroxine treatment with levels above the suggested doses
for replacement therapy, was seen to be well tolerated and patients were observed to respond to the treatment. In
the present study, it is aimed to underline that thyroxine augmentation therapy can be applied with supraphysiological
levels in patients with resistant bipolar disorder. Furthermore, the treatment of a resistant bipolar disorder
patient with a coexisting congenital hypothyrodism, is reported for the first time in this study.
