Sertleşme Bozukluğu Olan Erkeklerde ve Eşlerinde Beden Algısı, Kişilik Özellikleri ve Cinsel Doyum

Amaç: Bu çalışmada sertleşme bozukluğu tanısı konan bireyler ile eşlerinin kişilik özelliklerinin, cinsel doyum ve beden algılarının depresif bireylerle karşılaştırılarak araştırılması amaçlanmıştır.
Yöntem: Çalışmaya sertleşme yetersizliği olan 26 olgu ile eşleri ve depresyon tanısı konan benzer sosyode-mografik özellikteki 11 kadın ve 11 erkek alınmıştır. Tüm olgulara Minnesota Çok Yönlü Kişilik Envanteri (MMPI), Golombok-Rust Cinsel Doyum Envanteri (GRISS), Vücut Algı Ölçeği (VAÖ) ve DSM-IV Eksen I Bozuklukları için Yapılandırılmış Psikiyatrik Görüşme Formu (SCID-1) uygulanmış, depresyon tanısı alan hasta grupları ise ayrıca Hamilton Depresyon Derecelendirme Ölçeği (HDDÖ) ile değerlendirilmiştir.
Bulgular: Sertleşme bozukluğu olan bireylerin depresif erkeklere gbre MMPI alt puanlarından K puanları yüksek; F, Hs, D, Hy, Mf, Pt, Sc ve Si puanları düşük bulunmuştur. Sertleşme bozukluğu olan hasta eşlerinin, depresif kadınlara oranla L ve K puanları yüksek; F, Pa, Sc ve Ma puanları ise düşüktür.
Cinsel doyum ve işlevleri değerlendiren GRISS verilerine gbre, genel anlamda bütün gruplarda cinsel alanda
sorunlar bulunmasına karşın, sertleşme bozukluğuna erken boşalma sorunları da eşlik etmiştir. Diğer taraftan, hasta eşlerinin "anorgazmi" puanları depresif kadınlara gbre yüksektir.
Sertleşme bozukluğu olan bireyler ile eşlerinin bedenleriyle ilgili algılamalarını değerlendiren Vücut Algısı Ölçeği puanları depresif gruplara oranla anlamlı düzeyde yüksektir. Buna karşın, cinsel organ ve işlevler açısından, sertleşme bozukluğu olanların puanları eşlerinden ve depresif erkeklerden anlamlı düzeyde düşüktür.
Sonuç: Sertleşme bozukluğu olanlar, genel anlamda nevrotik özellikler ve spesifik sorunlarının dışında, cinsel doyumsuzluk yönünden depresif erkeklerle benzeşmektedir. Cinsellikle ilgili beden algısı ise depresiflere göre olumsuzdur. Sertleşme sorunu olanlar kendilerinden hoşnut olmayan, karamsar, somatizasyona eğilimli, yetersiz ve güvensiz hisseden, onaylanma gereksinimi olan kişilerdir. Bu gereksinimleri karşılanmadığında, kızgınlıklarını uygun biçimde ifade edemeyip kendilerini ve eşlerini farklı şekillerde cezalandırabilirler. Bu hastaların eşleri ise savunucu özellikler göstermekte ve sorun kaynağının kendilerinden bağımsız olduğunu düşünmektedir. Bu verilere gbre sertleşme bozukluğu olan hastaların eşleriyle birlikte değerlendirilmesi ve tedavilerinin eşleriyle birlikte yürütülmesinin uygun olacağı sonucuna varılmıştır.
Personality Features, Body Image, and Sexual Satisfaction in Erectile Dysfunction Patients and Their Spouses
Objective: The aim of this study was to investigate the personality characteristics, body image, sexual satisfaction in patients with erectile dysfunction and their spouses, in comparison with depressive patients.
Method: 26 males with erectile dysfunction and theirs spouses were compared with 11 male and 11 female depressives matched in socio-demograp-hic characteristics. All subjects received the Minnesota Multiphasic Personality Inventory (MMPI), Golombok-Rust Inventory of Sexual Satisfaction (GRISS), Body Cathexis Scale (BCS) and were evaluated with Structured Clinical Interview for DSM-IV Axis I Disorders (SCID-1). Depressive patients were also assessed by Hamilton Depression Rating Scale (HDRS).
Results: Out of MMPI subscales, K scores were higher while F, Hs, D, Hy, Mf Pt, Sc and Si were lower in patients with erectile dysfunction compared to depressive men. When compared with depressive females, subscales L and K were higher and F, Pa, Sc and Ma scores were lower in the spouses.
All groups appeared to have some problems in sexual functioning according to GRISS, while "premature ejaculation" was also a problem to be considered in erectile dysfunction patients. GRISS subscales showed that "anorgasmia" scores of the spouses were higher than depressive females.
BCS scores showed that patients and their spouses were generally more satisfied with their body image than depressives, although erectile dysfunction group had significantly lower scores in items related to sexual organs and functioning compared to their spouses or depressive males.
Conclusion: Erectile dysfunction patients and depressive males have similar neurotic personality characteristics and, except for the specific dysfunction areas in patients with erectile dysfunction, both groups appear to be sexually dissatisfied in general. Also, the sexual body image of erectile dysfunction patients is more negative than other groups. These patients can be described as dissatisfied, pessimistic, somatizing individuals with feelings of inadequacy and insecurity. They are likely to need others' support and attention. When their needs are frustrated, they may be inclined to punish others as well as themselves. On the other hand, the spouses of these patients appear to be defensive and to attribute responsibility to others.
According to these findings, it can be concluded that patients with erectile dysfunction should be assessed and treated together with their spouses.
