Obsesif Kompulsif Bozukluk (OKB) birey tarafından irrasyonel bulunan, rahatsızlık duyulan, yineleyici, dürtü, düşünce, imge ve/veya kompulsiyonların varlığı ile belirli bir bozukluk olup, en sık rastlanan dördüncü psikiyatrik bozukluktur. Dini obsesyonlar; tanrıya küfretme, varlığına inanmama gibi günah sayılan düşüncelerin akla gelmesi ve bundan ötü- rü ceza almaktan korkma ile belirli obsesyonlardır. Cinsel obsesyonlar, herhangi bir kimse ile ilgili istenmeyen cinsel düşüncelere sahip olma, çocukları içeren cinsel düşünceler, eşcinsellik ile ilgili cinsel düşüncelere kapılma ile ilişkili ob- sesyonlardır. OKB bireyin işlevselliğinde ve yaşam kalitesinde düşüşe yol açmaktadır. Obsesyonların içeriğinin cinsel ve dinsel konularda olduğu durumlarda OKB’nin yol açtığı rahatsızlığa bir de utanç ve suçluluk duyguları eklenmekte ve hastanın durumu daha da güçleşebilmektedir. Sunduğumuz olguda DSM-5 tanı kriterlerine göre OKB tanısı almış, dinsel ve cinsel obsesyonları ve temizlik kompulsiyonu alt tiplerine sahip olan ve bu nedenle günlük yaşamında önemli düzeyde zorlanmalar yaşayan 16 yaşındaki bir ergen hastanın psikofarmakoterapi ve bilişsel davranışçı terapi ile yü- rütülen tedavi süreci ele alınmaktadır. Tedavi prosedüründe olgunun obsesyon, kompulsiyon, davranışsal nötrleme ve kaçınmaları için, farmakoterapinin yanında başta Bilişsel Davranışçı Terapinin (BDT) OKB’deki temel yaklaşım tekniği olarak bilinen maruz bırakma-tepki önleme müdahalesi olmak üzere BDT ile çalışılmıştır. Tedavi prosedürü sonunda danışanın tüm obsesyon ve kompulsiyonlarında belirgin azalma ve günlük işlevselliğinde belirgin artış görülmüştür. Sonuç olarak; başta maruz bırakma ve tepki önleme tekniği ile BDT teknikleri ile farmakoterapinin birlikte kullanımının OKB’nin belirti şiddetini önemli düzeyde azalttığı ve bireyin işlevselliğine katkıda bulunduğu belirlenmiştir.

Sexual obsessions are obsessions associated with having unwanted sexual thoughts about anyone, sexual thoughts involving children, sexual thoughts about homosexuality. OCD leads to a decrease in the functionality and quality of life of the individual. In cases where the content of the obsessions is about sexual and religious issues, fe- elings of shame and guilt are added to the discomfort caused by OCD, and the patient’s condition can become more difficult. In the case we present, the treatment process of a 16-year-old adolescent patient who was diagnosed with OCD according to DSM-5 diagnostic criteria, has religious and sexual obsessions and cleaning compulsion subtypes, and therefore has significant difficulties in his daily life, is handled with psychopharmacotherapy and cognitive beha- vioral therapy. In the procedure, CBT was used for obsessions, compulsions, behavioral neutralization and avoidance of the patient, besides pharmacotherapy, exposure-response prevention intervention, which is known as the main approach technique of CBT in OCD. At the end of the treatment procedure, a significant decrease in all obsessions and compulsions of the client and a significant increase in daily functionality were observed. As a result; It has been determined that the combination of exposure and response prevention technique, CBT techniques and pharmaco- therapy significantly reduces the severity of OCD symptoms and contributes to the functionality of the individual.
