İdame Elektrokonvülsif Terapinin Otizm Spektrum BozukluğundaAjitasyon ve Kendine Zarar Verme Davranışlarının TedavisindeKullanımı

Otizm spektrum bozukluğunda (OSB) kendine zarar verme davranışı,
sıklıkla tehlikeli doku yaralanmasına ve hayati risklere yol açabilecek
yıkıcı belirtilerden biridir. Bu tür durumlarda kendine zarar verme
davranışı ve saldırganlık, psikofarmakolojik ve davranışsal girişimlere
cevap vermeyebilir. Yazında ilaç tedavilerine yanıt alınamayan ve
elektrokonvülsif terapiyle (EKT) iyileşme görülen olgular yer almaktadır;
ancak fayda gören olgularda terapinin etkinliği, tedaviye nasıl devam
edileceği ve idame EKT (i-EKT) ile ilgili sınırlı bilgi mevcuttur. Bu
çalışmada OSB tanısı ile izlenen; ilaç tedavilerine yanıt vermeyen
belirgin ajitasyon, duygudurum bozukluğu, ağır kafa travmasına
neden olacak kendine zarar verme davranışları olan iki olgunun klinik
özellikleri, tedavi süreci ve i-EKT tedavisi sunulmuştur.
İlk tedavi aşamasında EKT 7 seans olarak planlanmış ve olguların 5.
seans sonrası iyileşme gösterdiği gözlenmiştir. Ancak taburcu edildikten
sonra devam eden ilaca rağmen bir hafta sonra tekrarlayan ajitasyon
sebebiyle i-EKT’nin uygulanmasına karar verilmiştir. İlk olguda
i-EKT’ye iki haftada bir toplam 46 seans, ikinci olguda ise haftalık
olmak üzere toplam 18 seans olarak devam edilmiştir. Tedaviye bağlı
belirgin bir yan etki ya da komplikasyon gözlenmemiş ve genel iyilik
hali devam etmiştir. Çalışmamız OSB tanısı konulan hastalarda görülen
ajitasyonun i-EKT ile tedavi edildiği kısıtlı sayıda olgulardan birisidir ve
sonuçlara dayanarak i-EKT, ilaca dirençli olgularda bir tedavi seçeneği
olarak düşünülebilir.
Maintenance Electroconvulsive Therapy for Agitation and SelfInjurious Behaviors in Autism Spectrum Disorder
Self-injurious behaviors (SIBs) in autism spectrum disorder (ASD)
are destructive symptoms that can lead to dangerous injuries and
life-threatening risks. Agitation and SIBs may not respond to
psychopharmacological and behavioral interventions. There are reports
in the literature on improvement after electroconvulsive therapy (ECT)
in cases not responding pharmacotherapy. However, data on the efficacy
of the therapy on the benefiting patients, the course of the treatment and
on the use of maintenance ECT (m-ECT) are very limited. This report
presents the clinical features and the course of m-ECT on two cases
under follow up for pharmacotherapy resistant ASD with significant
agitation, mood disorder and SIBs that could cause severe head traumas.
The initial stage of therapy consisted of 7 sessions of ECT patients
showed improvement after the 5th session. m-ECT were started
since the agitation repeated one week after discharge despite ongoing
pharmacotherapy. In the first case, m-ECT was continued once every
two weeks for a total of 46 sessions; and in the second case a total
of 18 weekly sessions were conducted. No significant side effects or
complications were observed and the general state of well-being was
preserved. Our paper is among the few that reported succesful treatment
of agitation with m-ECT. m-ECT should be considered in treatment
resistant cases.
