Sürekli olarak uyumlu bir narsisistik deneyim arayışında olan narsisitik-sapkın özne ötekinden bağımsız bir deneyim yaşayamadığından, ötekinden kendini korumalıdır. Mazoşistik aktarım ile süregelen terapötik süreçte öne çkan özellikleri şunlardır: Sapkın anlaşma veya sözleşme, bağın erotizasyonu, çerçeveye ve analistin zihnine saldırı, alay etme. Narsistik sapkınlığın çocukluktaki kökenlerinde ensestüel nitelikteki ebeveyn-çocuk ilişkileri öne çıkmaktadır.
