Ruhsal hastalıkların etkili bir şekilde tedavisi için doğru teşhis psikiyatrideki medical yaklaşımda olduğu kadar psikoterapide de önemlidir. Bunun için gerekli olan parametreler bazen rahatsızlığın gelişim sürecinde, bastırma ve inkâr savunma mekanizmalarının da etkisiyle örtük kalabilmektedir. Paranoid bozukluklar bunlardan biridir. Bunda kişiliğin hezeyanla ilgili kısmı haricinde büyük ölçüde yıkıma uğramaması, gerçeklik algısının fazla bozulmaması ve bireyin sosyal hayattan uzaklaşıp yalnızlaşması dışında işlevlerini büyük ölçüde yerine getirmesi etkili olmaktadır. Bireyin hasta olduğunu kabul etmemesi sebebiyle tıbbi yardım almayı reddetmesi hastalığın altında yatan ruhsal süreçlerin de daha az araştırılmasında etkili olmaktadır. Psikanalitik yaklaşıma göre yansıtma savunma mekanizmasının yoğun olarak kullanıldığı paranoid bozukluklarda projektif teknikler hem hastalığı hem de sanrıların altında yatan ruhsal süreçleri daha iyi anlamaya yardımcı olmaktadır. Bu da hasta ve ailesinin uygun şekilde yönlendirilmesini ve daha etkili tibbi yardım almasını mümkün kılmaktadır. Bu çalışmada major depresyon ve yineleme bozukluğu ile tedavi gören daha sonra öldürüleceği sanrıları ile ilaç kullanmayı reddeden bir hastanın ailesini yönlendirmekte ve kendisini hastaneye gitmeye ikna etmekte projektif testlerin katkısı anlatılmaktadır.

The diagnosis of the psychological illnesses is very important for the efficient medical treatment. in psychiatry and for psychotherapy. Sometimes the parameters needed for this remains covered due to defense mechanisms such as suppression and denial during the development of the illness. The paranoid disorder is one of these illnesses. One of the causes of this is that the individual seems mostly functional in his/her daily life and the personality is not destructed except the delirium side and alienation socially. Since the individual doesn't accept the problem and so the medical treatment the psychological processes underlying are not examined. According to psychoanalytic approach, projection is used in paranoid disorder. So projective techniques help to understand better the psychological processes underlying the disorder and delusions. Thus this makes possible to direct the patient and the family for more efficient treatment. In this study, it can be found the case of a patient which treated medically with the diagnosis of major depression and obsessive disorder. She then rejects the medical treatment due to delirium which claims that her family will kill him with these medications. The contribution of the projective tests Rorschach and T.A.T. towards a better understanding of the illness can be seen.
