Behçet Hastalığı Olan Hastalarda Anksiyete ve Depresyonun Psoriasis Hastalarıyla Karşılaştırılması

Amaç: Behçet Hastaları ile psoriasis hastalarının anksiyete ve depresyon yönünden karşılaştırılması planlanmıştır
Yöntem: Çalışmaya Gazi Üniversitesi Tıp Fakültesi Dermatoloji Bölümünde Behçet Hastalığı tanısı ile izlenen 112 hasta ile yine aynı klinikte psoriasis tanısı ile izlenen 95 hasta alınmıştır. Hastalara konsultasyon-liyezon psikiyatrisinde görevli bir psikiyatrist tarafından Beck depresyon envanteri ve Beck anksiyete envanteri verilmiş ve demografik bilgileri kaydedilmiştir.
Bulgular: Behçet hastalarında Beck depresyon envanteri puanlan ortalaması psoriasis grubuna göre anlamlı olarak yüksek bulunmuştur(p< 0.001). Benzer şekilde Beck anksiyete puanlarının ortalaması Behçet hastalığı olan grupta psoriazisi olan gruba göre istatistiksel olarak anlamlı dercede yüksek bulunmuştur (p< 0.001)
Tartışma: Geniş sayılabilecek bir örneklemle yapılan bu çalışmada, Behçet hastalarında anksiyete ve depresyon belirtilerinin psoriasis hastalarına göre anlamlı derecede yüksek olduğu gözlenmiştir. Bu bulgu daha önce yapılmış olan çok daha küçük örneklemli benzer bir çalışmanın bulgularını desteklemektedir. Behçet Hastalarında anksiyete ve depresyon belirtilerinin yüksek oluşu, Behçet hastalarının tanı ve tedavisinde multidisipliner yaklaşımda psikiyatrik yaklaşımın dahil edilmesinin gerekliliğini göstermektedir. Bu şekilde hastaların yaşam kalitelerini artırabilmek mümkün olacaktır. Behçet hastalarında, psoriasis hastalarına göre daha fazla depresif ve anksiyöz belirtilerin görülmesi her iki hastalığın doğasındaki farklılıktan kaynaklanıyor olabilir. Bu durumun aydınlatılması ancak Behçet hastalarındaki psikiyatrik semptomların etiyopatogenezin anlaşılabilmesi ile mümkündür. Bu konuda ileri çalışmalara ihtiyaç vardır
Anxiety and depression in Behcet's Disease patients compared to psoriasis patients
Objective: The aim of this study is to evaluate depression and anxiety levels in BD patients and compare the findings with the findings of patients with psoriasis.
Method: Patients were collected from the dermatology department of Gazi University Faculty of Medicine. 112 patients with Behcet disease and 95 patients with psoriasis were enrolled in the study. Patients were evaluated by Beck depression and Beck anxiety inventory. Results were analysed by SPSS package program version 11.0
Results: There was no statistically significant difference between two groups in terms of age and disease period. The group with Behcet's di¬sease had higher scores both in Beck's depression inventory and Beck's anxiety inventory compared to the group with psoriasis (p< 0.001)
Discussion: Behcet disease is a chronic and episodic disease. Systemic vasculitis is generally considered as the basic pathological condition of the disease. Major symptoms observed in the disease are apthous ulcerations of the oral mucosa, genital ulcers and iridocyclitis with hypop¬yon, but the clinical pictures are variable among patients. The course of the disease is often devastating. Neurological involvement in BD was first defined in 1941 and when the central nervous system is affected by the disease, some psychiatric symptoms may accompany the disease. Lately, cognitive impairment without CNS involvement has also been defined. Neither the real relationship between psychiatric symptoms and BD, nor the role of emotional stres in BD has not been clarified yet. Higher levels of depression and anxiety in Behcet's Disease compared to patients with psoriasis may result either from the nature of the disease which leads to CNS involvement or episodic chronic nature which is devastating. But the etiopathogenesis of depression and anxiety in BD is stil unclear. Further researches with sophisticated imaging tecniques are needed to identify the etiopathogenesis of anxiety and depression in Behcet's Disease.
