Obstrüktif Uyku Apne Sendromunda Kronik İntermitant Hipoksemi Periferik Sinir Motor Liflerde (Ünit Sayısında)Azalmaya Neden Olur

Amaç: Obstrüktif Uyku Apne sendromu (OSAS) kronik tekrarlayıcı
hipoksik süreçtir. Bu çalışmada OSAS tanısı ile izlenen olgularda kronik
hipoksinin iskelet kası motor ünit sayısında yaptığı değişiklikler araştırıldı.
Gereç ve Yöntem: Apne-hipopne indeksine (AHİ) göre hastalar 2 gruba
ayrıldılar [grup 1: AHİ <15 (NK) ve grup 2: AHİ ≥15,1]. Grup 1’de 57
ve grup 2’de 62 hasta çalışmaya dahil edildi. Hastaların N. median
ve N. ulnarisin motor ve duysal distal latansları, ileti hızları ve birleşik
aksiyon potansiyel amplitüdleri değerlendirildi. Hastalarda submaksimal
uyarımla maksimum M yanıtları elde edildikten sonra motor ünit sayıları
(MUNE) Keypoint cihazındaki program ile elde edildi.
Bulgular: Her iki grup arasında N. medianus, N. ulnaris motor ve duysal
sinir distal latansları, maksimum M yanıt amplitüdü ve iletim hızları
ortalaması arasında fark yoktu (p>0,5). N. medianusa ait maksimum
M alan ortalaması grup 1’de 50,8±20,4 (16,1-121,7), grup 2’de
48,6±20,05 (10,5-111,4) saptandı (p=0,55). MUNE ortalaması grup
1’de 155,3±41,17 (46,6-251,7) ve grup 2’de 127,7±40,2 (22,8-235)
(p<0,00) olarak belirlendi.
Sonuç: OSAS hastalarda, MUNE yöntemi ile motor ünit sayısında anlamlı
düzeyde azalma, uyku sırasında tekrarlayan kronik hipokseminin OSAS
hastalarında MUNE’de azalmaya ve subklinik polinöropati için bağımsız
bir risk faktörü olduğunu göstermektedir.
Chronic Intermittent Hypoxemia in Patients with Obstructive Sleep Apnea Syndrome Causes Reduction of Peripheral Nerve Motor Fibers (Unit Number)
Objective: Obstructive Sleep Apnea syndrome (OSAS) is a chronic
intermittent hypoxic process. In this study, we aimed to investigate
electrophysiologically the changes in number of skeletal muscle motor
unit due to chronic intermittent hypoxemia in cases followed up with
the diagnosis of OSAS.
Materials and Methods: According to the apnea-hypopnea index (AHI),
patients divided into two groups (group 1: AHI <15 and group 2: AHI
=15.0). Fifty-seven patients in group 1 and 62 patients in group 2 were
included in the study. Motor and sensory distal latencies, conduction
velocities and combined action potential amplitudes of nervus medianus
and nervus ulnaris were evaluated in patients. After obtaining maximum
M responses by submaximal stimulation in the patients, motor unit
numbers (MUNE) were achieved with the program in Keypoint device.
Results: There were no statistical differences between the two groups in
terms of nervus medianus and nervus ulnaris motor and sensory nerve
distal latencies, maximum M response amplitude and mean conduction
velocity (p>0.5). Maximum M mean area of nerve medianus was
50.8±20.4 (16.1-121.7) in group 1 and 48.6±20.05 (10.5-111.4) in
group 2 (p=0.55). The mean MUNE values were 155.3±41.17 (46.6-
251.7) in group 1 and 127.7±40.2 (22.8-235) in group 2 (p<0.00).
Conclusion: The MUNE method demonstrates that chronic intermittent
hypoxemia during sleep causes reduction of the motor unit number
in OSAS patients and is an independent risk factor for subclinical
polyneuropathy.
