NMS antipsikotik ilaçlara bağlı gelişen, yaşamı tehdit eden sık görülmeyen bir sendromdur. Elektrokonvulsif tedavinin (EKT), NMS için etkili bir tedavi şekli olduğu bilinmektedir. Bu yazıda, ikiuçlu bozukluk hikayesi olan 37 yaşında bayan hastada kas içi tek doz klorpromazin uygulaması sonrası gelişen nöroleptik malign sendrom (NMS) ve elektrokonvulsif tedaviye olumlu yanıtı incelenmiştir. Hastanın herhangi bir başka psi-kotrop ilaç kullanımı yoktu. Hasta son 24 saatte ilerleyen ağır kas sertliği, ateş yüksekliği, terleme, kan basın¬cı değişikliği, mutizm ve bilinç değişikliği ile gelmişti. Kreatin fosfokinaz (CK) düzeyi 5064 U/L idi. Bu vakada NMS, genel anestezi altında günde bir olmak üzere toplam yedi adet EKT uygulaması sonrasında başarılı bir şekilde tedavi edilmiştir. Günlük EKT sık kullanılmamaktadır, ancak bu şekildeki bir uygulama ile NMS gibi yüksek ölüm riski olan hastalıkta kısa süre içerisinde iyileşme ve daha iyi sonuç alınması mümkün olabilir

NMS is an uncommon but potentially life-threatening adverse side effect associated mostly with typical antipsychotic agents. Electroconvulsive treatment (ECT) is known to be effective in the treatment of NMS. We present a case, a 37- [year]s-old female with a history of bipolar disorder (BD) who developed signs and symptoms associated with Neuroleptic Malignant Syndrome (NMS) after one day of treatment with single dose intramuscular chlorpromazine. The patient was not on any other psychotropic medication. Presented with severe muscle rigidity, elevated body temperature, tremors, labile blood pressure, diaphoresis, mutism and mental status changes that had progressed over the preceding 24 hours. Laboratory data revealed an escalating creatinine phosphokinase (CK) up to 5064 U/L In this case report, NMS successfully treated with daily seven bilateral ECT under general anesthesia. Daily application of ECT is not commonly used but it may be a better and emergent way to cope with a highly mortal state as NMS in a short time period.
