DSM-III-R ile birlikte, klasik obsesif kompulsif nevroz tanımından temel olan “belirtilere yönelik içgörünün tam olması” koşulu ortadan kaldırılarak, obsesif kompulsif bozukluk tanısı “içgörüsü az olan” olguları da içine alacak şekilde genişletilmiştir. Bu çalışmaya, DSM-III- R ölçütlerine göre obsesif kompulsif bozukluk tanısı konmuş, fiziksel hastalığı ve psikotik bozukluğu olmayan 42 hasta alındı. Hastalar, Yale Brown Obsesyon Kompulsiyon Ölçeği’nin 11. sorusuna göre içgörüsü tam olanlar (0 puan alan 19 hasta), içgörüsü değişik derecelerde az olanlar (1-4 arasında puan alan 23 hasta) olmak üzere iki gruba ayrıldı. İki grup arasında, sosyo-demografik özellikler, diğer psikiyatrik bozuklukların eşlik etme oranları, anksiyete ve depresyonun şiddeti, obsesyon ve kompulsiyonların sıklığı açısından (içgörüsü az olan grupta cinsel obsesyonların daha sık görülmesi dışında) anlamlı bir fark saptanmadı. Obsesyon-kompulsiyon puanları, içgörüsü az olan grupta anlamlı biçimde yüksek bulundu. İçgörü derecesi ile obsesyon-kompulsiyon puanları arasında anlamlı korelasyonlar saptandı. Bulgular, obsesyon ve kompulsiyonlara yönelik içgörüdeki bozulmanın, belirtilerin şiddetiyle ilişkili olduğunu göstermektedir.

Begining with DSM-III-R, the condition of an intact insight towards obsessive compulsive symptoms, which was essential in the classical definition of obsessive compulsive neurosis, has been removed, permitting the inclusion of cases with poor insight. A total of 42 patients who had no physical disease and psychotic disorder, and who had been diagnosed as having obsessive-compulsive disorder according to DSM-III-R criteria were included in this study. Acording to item 11 of Yale-Brown Obsessive Compulsive Scale, the patients were divided into two groups: one group comprising 19 patients with intact insight (having a score of 0) and the other comprising 23 patients with poor insight in different degrees (having scores of 1 to 4). The subgroups had no significant difference in terms of sociodemographic characteristics, comorbidity of psychiatric disorders, severity of anxiety and depression, and the frequency of specific kinds of obsessions and compulsions, with the exception that the patients with poor insight were more likely to have sexual absessions. Obsession compulsion total scores were significantly higher in the group with poor insight. Statistically significant correlations were found between the degree of insight and total obsesion compulsion scores. Our findings suggest a relationship between impairment of insight and severity of symptoms in obsessive compulsive disorder.
