Yeme Bozukluğu Olan Ergenlerin Sosyodemografik, Ailesel Ve Kişisel Özellikleri

Amaç: Bu çalışmada lise öğrencisi ergenler arasındaki yeme bozukluğu olgularının belirlenmesi; olgu grubunun sosyodemografik, kişisel, ailesel, yaşantılara ve alışkanlıklara bağlı özelliklerinin incelenmesi ve bu özelliklerin normal kontrol grubuyla karşılaştırılması amaçlanmıştır.
Yöntem: İstanbul, Bakırköy’de küme örneklemesi yoluyla seçilen dört lisenin öğrencilerine Yeme Tutum Testi verilmiş ve kesme puanı üzerinde puan alan öğrencilerle DSM-III-R-Yapılandırılmış Klinik Görüşme Formu kullanılarak tanısal görüşme yapılmıştır. Kontrol grubu aynı liselerdeki öğrencilerden test puanı düşük olan ve tanı almayan öğrenciler arasından seçilmiştir. Olgu ve kontrol grubundaki tüm öğrenciler için ayrıntılı bir genel bilgi formu doldurulmuştur. Her öğrencinin boyu ve kilosu aynı ölçüm aletleri kullanılarak belirlenmiştir.
Bulgular: Yeme Tutum Testini doğru olarak dolduran 944 öğrenciden ölçek puanı 30 ya da üzerinde olan 101 öğrenciyle yapılan tanısal görüşmelerde 25 öğrenciye yeme bozukluğu tanısı konmuştur. Bu 25 öğrenciden 17’si bulimia nervoza, 8’i tıkınırcasına yeme bozukluğu tanısı almıştır. Olgu grubunun normal kontrol grubuyla karşılaştırılması sonucunda ortalama kilo, okula isteksizlik, evden kaçma davranışı, menstrüasyon düzensizliği gibi değişkenlerdeki farklılıklar istatistiksel olarak anlamlı bulunmuştur.
Sonuç: Ergenler arasında yeme bozukluklarının nadir olmadığı görülmektedir. Yeme bozukluklarının gizlilik içinde var olması, tedavi için başvuru oranının düşüklüğü ve uzun hastalık süresinin prognozu olumsuz etkilemesi olguların erken tanınmasının ve psikiyatrik yardım sağlama yolları konusunda aydınlatılmasının önemini ortaya koymaktadır.
Sociodemographic, Familial and Personal Characteristics of Adolescents with Eating Disorders
Objective: The aim of this study is to detect cases of eating disorders among high school students and then compare the cases with normal controls on sociodemographic, familial and personal variables.
Method: Eating Attitudes Test was given to the ninth graders of four high schools in Istanbul, Bakirköy. The students who scored equal to or above the cut-off point were interviewed using Structured Clinical Interview for DSM-III-R. Case group included students who had any eating disorder diagnosis. Control group consisted of the same number of students matched for class and sex. The students in the control group had test scores below the cut-off point. A detailed questionnaire prepared by the investigators was filled for all the students, and for each student, weight and height were measured using the same meters.
Results: 944 students filled Eating Attitudes Test correctly and among them 101 scored 30 or above. After diagnostic interview 25 students had an eating disorder diagnosis. Among these 25 students, 17 had the diagnosis of bulimia nervosa and 8 had binge eating disorder. When case group compared with control group, statistically significant differences were found between them with regard to mean weight, reluctance to go to school, running away from home and having menstrual irregularities.
Conclusion: Eating disorders are not infrequent among adolescents. The most frequent eating disorder was bulimia nervosa; girls are in greater risk for eating disorders. That eating disorders exist in secrecy, the rate of treatment seeking is low and the duration of disorder is reversly correlated with prognosis underscores the importance of earlier identification of cases and help them obtain psychiatric help when necessary.
