Mental Bozuklukların Tanısal ve Sayımsal Ölçütleri El Kitabının (Diagnostic and Statistical Manuel of Mental Disorders, DSM) ön gördüğünün aksine, farklı kuramcılar, narsistik kişilik bozukluğu kapsamında değinilen patolojik narsizmin çok boyutlu bir kavram olabileceğini öne sürmüşlerdir. Ancak önceki basımlarda olduğu gibi en son basım olan DSM-V’te (2013) de narsistik kişilik bozukluğu tanı kriterlerinde patolojik narsizmin büyüklenmeci boyutuna aşırı vurgu yapılıp kırılgan boyutuna değinilmemiştir. Bu gözden geçirme makalesinde patolojik narsizmin iki genel boyutuyla (büyüklenmeci ve kırılgan narsizm) değerlendirilmesi amaçlanmıştır. Bu amaç doğrultusunda literatürdeki patolojik narsizm ile ilgili araştırmalar taranmış, araştırma bulgularına patolojik narsizmin iki boyutu genel özellikleri, gelişimsel kökenleri, değerlendirme/tanı koyma süreçleri bakımından karşılaştırılarak yer verilmiştir. Son olarak, patolojik narsizm kavramını tam olarak kapsamak, patolojik narsizmin değerlendirme ve tedavi sürecine ışık tutmak amacıyla narsistik kişilik bozukluğu tanı kriterleri kapsamında yapılabilecek olası değişiklikler tartışılmıştır.

As opposed to the suggestions of DSM (Diagnostic and Statistical Manual of Mental Disorders) about pathological narcissism which is considered within the scope of narcissistic personality disorder, theoreticians emphasize that pathological narcissism is a multidimensional construct. However, in DSM-V, which is the latest edition, the grandiose subtype of pathological narcissism overemphasized among the diagnostic criteria but the vulnerable subtype was not mentioned as it is in the previous editions. Therefore, the purpose of the current review is to investigate pathological narcissism with its both dimensions which are grandiose and vulnerable narcissism. Parallel with the purpose of the review, in this article both subtypes of pathological narcissism are examined and compared for their general qualities, developmental roots and measurement/diagnosis processes with the help of the studies about pathological narcissism in the literature. Lastly, for the definite representation of pathological narcissism in narcissistic personality disorder diagnostic criteria and to enhance the evaluation and treatment processes of narcissistic personality disorder, some changes are suggested and discussed, respectively.
