Breuer, Viyana'da 19. Yüzyılın ikinci yarısında tanı koymada usta ve yeni teknikler deneme konusunda korkusuz bir hekimdir. Nietzsche, o yüzyılın keskin zekalı ancak bir o kadar da hakettiği ünü görecek kadar yaşayamayacak ölçüde hasta ve yorgun filozofu. Kitabın kurgusu sadece bu ikisinin birbirlerini tedavi etmeleri sürecini yaratarak, yazarın "varoluşçu psikiyatri"özlemini ortaya koymakla kalmamış; aynı zamanda hasta ve hekim arasındaki ilişkiden yola çıkarak her iki "aktörün" ve bunların kişilikleri ile yaşam deneyimlerinin de tanı ve tedavide karşılıklı olarak sürece nasıl katılabileceklerinin çarpıcı bir örneğini sergilemiştir. Üstelik roman, bu karşılıklı etkileşimde, bazı bilinen ve benimsenen etik kuralların çiğnenmiş olduğu, ancak hastanın, tıp etiğinin kapsamındaki süreçten daha geniş bir çerçeve içinde ve öngördüğünden de fazla boyutta yarar gördüğü bir senaryo imgelemiştir. Bu süreçte, ayrıca, bir tıp adamı ile bir felsefecinin düşünce ve karar verme işlev bütünlüğünün, her iki tarafın da kullandıkları farklı yaklaşım biçimlerine (teknik tıp yaklaşımı ve uygulamalı felsefe) karşın, nasıl oluşturulduğu da bu senaryoya dahil edilmiştir. Irwin Yalom'un bu yapıtındaki senaryonun 'bana düşündürdüklerini' günümüzde somutlaştırırsak konuyla ilgili olarak şunları söyleyebiliriz. Yukarıdaki tanıma, güvenme ve belirleyip tartışma sürecinde, yalnızca günümüzün "sağlık ekibi" ve "etikçi"si değil, aynı zamanda başvurulduğu anda devreye girerek katkıda bulunması düşünülen psikolog, psikiyatrist, din adamı, pedagog, hasta yakınları da gündeme getirilebilir. Örneğin bir hastanın belli fiziksel belirtilerinin ya da kişiliksel davranış bozukluğunun temelinde, kendisini anlatış biçiminden doğan uyumsuzluk ve bundan doğan korkular yatıyor olabilir. Bu durumda, kişinin yaşama bakışını anlamak ve bunu onunla konuşmak, herhangi bir tedavi yöntemine paralel olarak yapılabilir; ancak kim bu konularda tek başına yetkindir? Hastayı bu duruma getiren etkenlerin temelleri, değerler sistemine gömülü olarak yüzyıllar önce atılmış olabilir. Yüzyıllar boyunca düşünürler bunları tartışmış olabilirler.Belki hastanın ailesi bu konuların üzerine gitmekten çekinmiş ve bunları tabulaştırmış, hatta onunla aynı inanç ve korkulara sahiptir. Hastanın yakın ilişkide bulunduğu kişilerin davranışları bu uyumsuzluğu daha da belirginleştirmiş olabilir. Breuer'in romanda Nietzsche'nin migrenini babasının ölümüne ilişkin "yüzleştirme"yöntemiyle tedavi etmesinin, 21. Yüzyıla girdiğimiz bir dönemde belki pek tartışılacak ve ilginç bir yönü bulunmayabilir. Nietzsche'nin de karşılığında Breuer'in çektiği bazı fiziksel ve ruhsal sıkıntıların nedenini "ölüm korkusu" olarak düşünmesi ve hastayı, yani doktoru şifaya kavuşturması da birtakım Amerikan ya da Yeşilçam filmlerindeki kurgulara benzetilebilir ve gerçekçi bulunmayabilir. Hatta doktorun hastayı tedavi ediyormuş gibi görünmezken hastanın doktoru tedavi etmesi ve bunu da 19. Yüzyıl felsefesinin bir dehası olarak "kendi" teknikleriyle ve yöntemleriyle becermesi sadece bir roman konusu olmakla da kalabilir. Burada tartışılan, yazarın ne demek istediği ve istemediği de değildir. Tartışma konusu yazarın varoluşçu psikiyatri özlemi ile başarılı bir psikoterapinin senaryosunu oluşturduğu yapıtında kurguladığı hasta-hekim ilişkisini ilgilendiren kurgunun, tedavi süreçleri ve hasta tanımlaması konularında neler düşündürebildiğidir. Bu yazının temel konusu da bu kurgunun bir tıp insanı ve etikçi olarak yazara düşündürdükleridir. Bu ve buna benzer örnekler, herhangi bir insanın organik hastalıkları karşısında, sağlık felsefesinin, hangi bilim dalını tıpta tek başına sağaltıcı olarak yeterli görüp de tedavi sürecine uyumlayabileceği konusunda derin kuşkular uyandırmaktadır. Belki de bu konuyu tıp etiği ya da tıbbi bakış açısıyla değil bir tıp felsefesi ya da daha geniş boyutta bir "sağlık felsefesi" çerçevesinde ele almak gerekir. Bu konu yalnızca hastalıkların kişisel düzeyde ruhsal temelleri başlığı altında modern psikiyatrinin çabalarıyla değil de, içinde hastalıkların toplumsal bazda psikolojik ve ruhsal kökenlerinin de mercek altına yatırılabileceği yeni bir "sağlık süreci"nin tanımlanmasına yönelik bir model arayışı içinde ele alınabilir. Hastayı oluşturan kimliğin, yani hasta bireyi oluşturan özdeğerler, kabullenilmiş ahlaki normlar, psikososyal gelişim ve hastanın duygularının tanınması, bunun hasta ve doktor arasında tartışılarak paylaşılması belki de hastalık olgusunun her bir hasta için farklı bir betimleme (patientology) olduğu düşüncesini de birlikte getirmektedir. Disiplinlerarası olan tıp etiği ile normatif etik anlayışının bir adım sonrası olan çağdaş bir tıp etiği bekleyişi ve arayışının da ötesinde, ahlaki normların, değer yargılarının ve bunların psikososyal gelişim ve psikiyatri ile etkileşiminin birlikte çözümlemesine gereksinim duyulduğu kanısındayım.

In Vienna, during the second half of the 19th century, Breuer is an ingenious physician in diagnosis as he is brave in trying out novel techniques. Nietzsche, on the other hand, is one of the sharpest thinkers of the century, as he is most ailing and exhausted person so that he was not expected to live long enough to see the reputation he had possibly deserved. The story not only unveils the author's dream of "existentialist psychiatry" by placing the two in a process of "bilateral healing," but also, starting with the doctor-patient relationship, demonstrates in a striking way how bothplayers, their personalities, and life experiences can be mutually incorporated into thisprocess. Remarkably, the novel has envisioned that some known basic ethical principles have been marched on during this interaction; and "both" patients have benefited from it on a scale which is even more than what the medical ethics would expect. lt is outstanding that, during the above mentioned interactive period, the integrity of the processes of mutual intellectualization and decision making ofa "physician" and a "philosopher" are built up, via his technical medical approach by the former, and applied, through hisphilosophy, by the latter. In a scenario like this, indeed, the treatment of disease is such a complicated process that cannot, apparently, be left to the medical specialist alone. It seems important that the physicians, medical professionals and the patient know and trust each other. The definition of the specific and general foundations of the individual, social and psychological conditions that result in the disease become crucial here as well as the affirmation of these by the physician, patient, family, and other "players" that have role in the formation of these conditions. What this work of Irwin Yalom makes me think can possibly be adopted to our time in the following way. The process of acquaintance, trust, affirmation and discussion described above may not necessarily call only for today's "health team " and "the ethicist," but also for psychologists, psychiatrists, religious figures, pedagogues, the family and friends, thinkers, and social workers. For example, certain physical symptoms or the personality disorder of a patient may be due to a conflict that is caused by the patient 's way of expressing the self, and the anxiety that is thus created. In such case, parallel to any method of treatment, an attempt at understanding the patient's perception of life and discussing it with him/her can be really useful. But who is so competent as to do this all alone? Origins of the factors that created the illness may bed in the system of values which may go back to hundreds of years. Thinkers may have been discussing these conditions and describing them. The family of the patient may have avoided understanding these subjects and tabooed them, and they even may posses the same beliefs or fears. Close contacts with the patient may have made the patient's conflicts more evident. Breuer's healing of Nietzsche's migraine by employing a method of ''confrontation" may not constitute much ofan interest and point of discussion on the threshold of the 21st century. Likewise, Nietzsche’s discovery of the "fear of death " as the cause of the physical and spiritual disorders that Breuer experiences, and his healing a doctor may be compared to certain movies and thus considered unrealistic. Furthermore, the patient's treating the physician with his "own" techniques and methods, as a deviant genius of the 19th century philosophy, while the latter is pursuing on his part an undercover therapy for the former, might be a best-seller theme. However, this is not the point that the author wishes to make. The concern underlying this article is what this scenario might make a medical professional and an ethicist think. Examples like the one in this book bring about profound skepticism on what scientific discipline the medical professional can regard as a self-sufficient healer and accommodate to the process of treatment. Perhaps the subject must be embraced not in the context of medical practice and/ or medical ethics, but from the viewpoint of medical philosophy, or in a broader realm of "philosophy of health." indeed, this topic can be discussed not only with the lahor of modern psychiatry, and not in the class of mental origins of diseases on an individual level. It can be refined in the pursuit of a model for defining a new "process of health" in which the collective sources of diseases on a societal and moral basis can be laid under lenses. There are now high hopes and expectations for a new medical ethics that is considered as a step forward from the modern multidisciplinary medical ethics and the normative ethical stand point. But even much beyond the broad limits of this conception, there is need for a greater model of "definition of diseases and treatment in the broader context" that will include philosophy, social and administrative sciences, and art as well as medicine.
