Otizm spektrum bozukluğu (OSB) kendine özgü davranışsal belirtileri olan bir nörogelişimsel bozukluktur. DSM-5'e göre çekirdek klinik belirtiler, sosyal iletişim ve etkileşimde yetersizlik; sınırlı, tekrarlayıcı davranış özellikleri ve ilgiler, olmak üzere temel iki kümede ele alınır. Birçok çalışma, özellikle ikiz ve aile çalışmaları, OSB'de genetik etmenlerin önemini vurgulamaktadır. OSB'nin kalıtılabilirliği %90'ın üzerinde olsa da, etiyolojiye etki eden genetik etmenler heterojendir. Kalıtım biçimi belirlenememiştir. Birçok çalışma OSB etiyopatogezinde çevresel etmenlerin de rolünü vurgulamaktadır. Bu çevresel etmenler nörogelişimden sorumlu genlerin ifadesini etkileyebilmektedir. Bunun yanında OSB etiyopatogenezinde epigenetik düzeneklerin de olasılıkla rolü bulunmaktadır. Bu yazıda OSB etiyopatogenezinde genetik, çevresel ve epigenetik etmenlerin rolleri gözden geçirilmesi amaçlanmıştır.

Autism spectrum disorder (ASD) refers to a neurodevelopmental condition defined behavioral features. According to DSM-5, the core clinical characteristics of ASD include impairments in two areas of functioning such as social communication and social interaction, as well as restricted, repetitive patterns of behavior, interests or activities. Evidence for the importance of genetic factors in the etiology of autism comes from many sources, including twin and family studies. Although the heritability of autism has been estimated to be as high as 90% genetic factors,that those are heterogeneous and polygenic. Despite advances in epidemiological, molecular and clinical genetics, the genetic risk factors remain difficult to identify. Several studies suggest a role of environmental factors ASD. Environmental factors can modify the expression of genes, besides epigenetic mechanisms has potential role in the development of ASD.
