Amaç: Çalışmamızda suç işlemiş erkek şizofreni hastalarında madde kötüye kullanımının, suç ve şiddet davranışı ile hastalığın klinik bulguları üzerine etkilerinin araştırılması amaçlandı. Yöntem: Bakırköy Ruh Sağlığı ve Sinir Hastalıkları Eğitim ve Araştırma Hastanesi Adli Psikiyatri Birimi’nde işledikleri bir suç nedeniyle yatarak gözlem veya koruma ve tedavi amacıyla bulunan, DSM-IV tanı ölçütlerine göre şizofreni tanısı konmuş erkek hastalar çalışmaya alındı. Hastalardan yaşamının bir döneminde ve/veya şu anda DSM-IV-TR tanı ölçütlerine göre madde kullanım bozukluğu bulunanlar belirlenerek eştanısı olan (s=50) ve olmayan (s=50) iki grup Sosyodemografik Veri Formu, Pozitif ve Negatif Sendrom Ölçeği (PANSS), İçgörünün Üç Bileşenini Değerlendirme Ölçeği (İÜBDÖ) ve Taylor Suç Şiddet Derecelendirme Ölçeği ile değerlendirildi. Bulgular: Çalışmamızda madde kötüye kullanımı eş tanısı olan şizofreni hastalarının %90 oranında paranoid alt tipte olduğu saptandı. Eş tanı durumunda işlenen suç sayısının fazla olduğu, ciddi şiddet içeren insan öldürme ve öldürme girişimlerinin olduğu, daha çok bıçak ve tabanca kullanıldığı belirlendi. Madde kullanan grubun %88’i suç sırasında madde etkisinde olduğunu, esrar ve karışık madde yeğlediklerini belirtti. PANSS puanları madde kullanan grupta daha yüksekti. Çocukluk çağında fiziksel şiddete maruz kalma, intihar girişimi ve kendine zarar verici davranışlar eş tanılı grupta daha fazla bildirildi. Sonuç: Şizofreni hastalarında madde kötüye kullanımı eş tanısı, psikopatoloji, şiddet davranışı, suç işleme davranışı ve tedavi uyumuna belirgin etkide bulunmaktadır. Şizofreni hastalarında madde ve suç arasında nedensel ilişkinin araştırıldığı çalışmalar, korumaya yönelik önlemler geliştirilmesinde de yardımcı olabilir

Objective: The aim of our study is to examine the effects of the substance abuse on criminal and violent behaviors, clinical features of the disease in criminal male with schizophrenia. Methods: Male inpatients with schizophrenia diagnosed according to the DSM-IV criteria who had been hospitalized for observation or mandatory treatment in Bakirköy Research and Training Hospital for Psychiatry, Neurology and Neurosurgery forensic psychiatry unit were participated in the study. Patients with substance use disorders according to the DSM-IV criteria at any time of their life and/or at that time were assessed, two groups as patients with (n=50) and without comorbidity were evaluated using the questionnaire for demographic and clinical characteristics, Positive and Negative Syndrome Scale (PANNS), Schedule for Assessing the Three Components of Insight and Taylor’s Violence Rating Scale. Results: In our study 90% of schizophrenia patients with substance abuse comorbidity were paranoid subtype. In case of comorbidity, number of committed crimes were higher, involved serious violence included homicide and attempted homicide using frequently knives and fire guns. Eighty-eight percent of the patients with substance abuse comorbidity stated that they used cannabinoid and several substances during these crimes. Their PANSS scores were higher. It had been found that they had experienced more physical violence at childhood, more suicidal attempts and self-injury behaviors. Conclusions: Substance abuse co-morbidity in schizophrenia markedly affects the psychopathology, violent behaviors, criminal behaviors and compliance with the treatment. Studies investigating the causal relationship between substance and crime in schizophrenia can be helpful in the development of methods targeting protection.
