Gerilim Tipi Başağrılarında Öfke Düzeyi, Öfke İfade Tarzı Ve Duygu Durum Profilinin Değerlendirilmesi

Amaç: bu çalışmada uluslararası başağrısı topluluğu (ubt) 2004 sınıflaması kriterlerine göre sık epizodik gerilim tipi başağrısı (gtba) ve kronik gtba tanısı konan hastalar ve kontrol grubu arasında öfke, öfke ifade tarzları ve duygudurum profillerinin karşılaştırılması amaçlanmıştır.
Yöntemler: çalışmaya ubt 2004 kriterlerine göre sık epizodik (n=28) ve kronik gtba (n=42) tanısı konan hastalar ve kontrol grubu (n=50) olarak da gönüllüler alındı. Katılımcılara sürekli öfke-öfke tarz ölçeği (söötö) ve duygudurum profili ölçeği (ddpö) uygulandı. Katılımcıların alt ölçeklerden aldıkları puanlar ayrı ayrı hesaplandı. Üç grubun yaş, eğitim düzeyi ve söötö ve ddpö alt ölçek puanları arasında farklılık olup olmadığı tek yönlü varyans analizi (anova) ile araştırıldı. Post hoc değerlendirme tukey testi ile yapıldı. Kategorik değişkenler karşılaştırılmasında ki-kare testi kullanıldı. P≤0.05 istatistiksel anlamlılık kabul edildi.
Bulgular: hasta gruplarının söötö'de sürekli öfke, öfke-iç ve öfke-dış puan ortalamaları arasında anlamlı fark yoktu. Ancak her iki grupta sürekli öfke ve öfke-iç puanları kontrol grubuna göre anlamlı derecede yüksekti (p<0.001), öfke-dış puanları ise kontrol grubuna göre anlamlı derecede düşüktü (p<0.001). Duygudurum profili değerlendirilmesinde; şaşkınlık, yorgunluk, anksiyete, depresyon ve öfke alt ölçeklerinden alınan puanlar her iki hasta grubu arasında farklı değilken kontrollere göre istatistiksel olarak anlamlı derecede yüksekti (sırasıyla p=0.014, p<0.001, p=0.011, p<0.001 ve p=0.003).
Sonuç: bu bulgular gtba hastalarında psikolojik ya da bedensel etkenlere bağlı olarak artmış olan öfkenin dışa yansıtılamadığını, bu nedenle içe dönük öfkenin artmış olduğunu, buna bağlı olarak da somatizasyon aracılığıyla başağrısı düzeyinin artabileceğini düşündürmektedir.
Evaluation Of Anger, Anger Expression Style And Mood Profile İn Tension-Type Headache
Objective: in this study, we aimed to compare anger, anger expression style and mood profiles between patients with frequent episodic tension-type headache (tth) and chronic tth, based on the international headache society (ihs) 2004 classification criteria, and control subjects.
Methods: patients diagnosed with frequent episodic (n=28) and chronic tth (n 42) according to the criteria of ihs 2004 and volunteers as control group (n=50) were enrolled in the study. The participants completed the state trait anger scale (stas) and the profile of mood states (poms). Subscale scores of participants were calculated separately. Age, educational level, and subscale scores of the stas and the poms were analyzed to observe whether differences exist between the three groups with one-way analysis of variance (anova). Post hoc evaluation was made with tukey's test. Categorical variables were compared using the Chi-square test. P≤ 0.05 was considered statistically significant.
Results: there was no significant difference between the patient groups with respect to mean scores of trait-anger, anger in and anger out of the stas. On the other hand, in both patient groups, trait-anger and anger in scores were significantly higher than those in the control group (p<0.001), while anger out scores were significantly lower than those in the control group (p<0.001). In mood profile evaluation, although there was no difference in scores of confusion, fatigue, anxiety, depression and anger Subscales between both patient groups, all were significantly higher than in the Controls (p=0.014, p<0.001, p=0.011, p<0.001 and p=0.003, respectively).
Conclusion: these findings suggest that patients with tth cannot express outward their anger, which is increased due to psychological or physical factors, so that inward anger can be increased in these patients; accordingly, the level of headache can be increased through somatization.
