Edinsel İşitme Kaybı ve Psikiyatrik Belirtiler

Amaç: Dünya Sağlık Örgütü (WHO) işitme kaybını, konuşmada olduğu kadar, algılamada, dikkat ve yönelimde de azalma olarak tanımlamıştır. Bu çalışmada, biyopsikososyal bütüncül görüşü temel alarak, genel hastane polikliniğine başvuran edinsel (ancak oluşum nedeni bilinmeyen) işitme kaybı ya da yetmezliği olan hastalardaki psikiyatrik belirti profili ve fiziksel hastalık ile psikiyatrik bulgular arasındaki ilişkiyi araştırmayı amaçladık.
Yöntemler: İşitme kaybı dereceleri hafif, orta ve ağır olmak üzere 3 ayrı hasta grubu ile bir sağlıklı kontrol grubu çalışmaya alındı. Her bir grup 30 kişiden oluşturuldu. Bir sosyodemografik anket formu, SCL-90-R ve SCID-NP uygulandı.
Bulgular: Çalışmamızda, genel psikopatoloji düzeyi kontrol grubunda %20 iken, işitme kayıplılarda %35.6 olarak saptandı. Hastaların %30-50'si işitme kaybının hem sosyal hem de ruhsal olumsuz etkilerinden yakındı. SCL-90-R'e göre, özellikle orta ve ağır derecede işitme kaybı olanlar daha fazla paranoid ve depresiftiler; daha fazla yeme ve uyku sorunları vardı; daha fazla müzikal varsanılardan (%25) yakınıyorlardı ve onların pozitif belirti toplamları ve ek ölçek puanları kontrol grubuna göre daha yüksekti. SCID-NP'de ise özellikle varsanı, paranoya ve depresyon skorları işitme kayıplı kişilerde daha yüksek olarak belirlendi.
Sonuç: İşitme güçlüğü ya da kaybı olan kişilerdeki psikiyatrik belirtilerin nedenleri çok etkenli olup, en önemli etyolojik etkenin yeti yitimi olduğu bilinir. Çalışmamızın sonuçları bize, psikiyatri ve diğer tıbbi alanların ortak çalışmasının her zaman gerekli olduğunu göstermiştir.
Acquired Hearing Loss and Psychiatric Symptoms
Objective: World Health Organization (WHO) defines hearing loss as an impairment in perception, attention and organisation as well as in speaking, In this study, on the base of biopsychosocial theory, we aimed to determine both psychiatric symptoms of the patients who have acquired (but ideopathic) hearing impairment/loss, and also, if there was, an association between this physical illness and psychiatric symptoms.
Methods: The groups of patients with mild, moderate and severe hearing impairment and a healthy control group were enrolled in the study. Each group contained 30 persons. A sociodemographic questionnaire, the Symptom Check List 90-Revised Form (SCL-90-R) and the Non-Patients Form of Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders (SCID-NP) were used.
Results: The psychopathology that have been found in our study was 35.6% in patients group versus 20% in controls. 30-50% of patients with hearing loss reported both social and psychological problems. According to SCL-90-R, in the patients? groups who had especially moderate and severe hearing loss were more paranoid and depressive; they had more eating and sleeping difficulties; also they complained more from musical hallucinations (25%) and their scores of positive symptoms and of additional scales in SCL-90-R were higher than the controls (p< 0.001). In SCID-NP, especially the hallucination, the paranoia and the depression scores were higher in patients with hearing loss.
Conclusion: The reasons for psychiatric symptoms in the patients with hearing impairment or loss are multiethiological and it is known that disability is the major ethiological factor. The results of our study showed us that the psychiatry and the other medical disciplines should always work together.
