Kronik Başağrısı ve Epilepsisi Olan Çocuklarda Psikiyatrik Eş Tanılar

Amaç: Bu çalışmada Gazi Üniversitesi Tıp Fakültesi Çocuk Nörolojisi Biriminde başağrısı ve epilepsi nedeniyle
izlenen hastaların komorbid psikiyatrik hastalıklar açısından incelenmesi amaçlanmıştır. Yöntem: Ocak
2008-Mayıs 2008 tarihleri arasında polikiniğe başvuran 7-18 yaş arası 74 hastaya DSM-4 e göre yapılandırılmış
görüşme ve Okul Çağı Çocukları İçin Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi-Şimdi
ve Yaşam Boyu Versiyonu (=K-SADS-PL) uygulandı.
Bulgular: 74 hastanın 59.4%'ü (n=44) kronik baş ağrısı, 40.6%'sı (n=30) epilepsi tanısı ile izlenmekte idi.
Başvuran hastaların 55.4%'ü (n=41) kız , 44.6%'sı (n=33) ise erkek idi. Olguların 44.6%'sı (n=33) herhangi
bir psikiyatrik tanı almadı. Olguların 43.2%'si ne (n=32) tek psikiyatrik tanı kondu. Bunlar anksiyete
bozukluğu (32.4%, n=24), depresif bozukluk (6.8%, n=5), dikkat eksikliği hiperaktivite bozukluğu(DEHB)
(4.1%, n=3) idi. Hastaların 12,2%'si (n=9) ikili psikiyatrik tanı aldı. Bunlar anksiyete-depresyon (6.8%,
n=5), DEHB-anksiyete (2.7%, n=2), DEHB-depresyon(2.7%, n=2) idi.
Sonuç: Bu çalışmada başağrısı ve epilepsi nedeniyle izlenen hastalarda depresyon ve anksiyete bozukluğunun
yaygın olduğu saptanmıştır. İstatistiksel olarak epilepsi ve başağrısı grupları karşılaştırıldığında komorbid
psikiyatrik hastalık görülme sıklığı açısından anlamlı fark bulunmamıştır(p=0.967). Ancak başağrısı grubunda
anksiyete bozukluğu, epilepsi grubunda DEHB diğerine göre daha sık rastlanmıştır.
Comorbid Psychiatric Diagnoses in Children with Chronic Headache and Epilepsy
Objective: The aim of this study is to identify comorbid psychiatric disorders in patients referred to Gazi University Faculty of Medicine Pediatric
Neurology Department with chronic headache and epilepsy.
Methods: The study group included a total of 74 patients (41 female and 33 male, ages ranging from 7 to18) who were refferred to the neurology
department between January ? May 2008. All patients were screened for psychiatric disorders with a structured interview form according
to DSM-IV criterias and Schedule for Affective Disorders and Schizophrenia for School Aged Children, Present and Lifetime Version
(K-SADS-PL).
Results: Of these 74 patients, 59.4% (n=44) were followed for headache whereas 40.6% (n=30) for epilepsy. 44.6% (n=33) of the patients
did not meet criteria for any psychiatric diagnosis. 43.2% (n=32) of the patients had only one psychiatric diagnosis. The most frequently
observed diagnosis were anxiety disorder( 32.4%, n=24), depression (6.8%, n=5), attention deficiency hyperactivity disorder (ADHD)
(4.1%, n=3). 12,2% (n=9) of the patients met criteria for multiple psychiatric diagnosis. These were anxiety-depression(6.8%, n=5), ADHDAnxiety
(2.7%, n=2) and ADHD-Depression (2.7%, n=2)
Conclusion: Our findings present the frequency of depression and anxiety disorders in patients with epilepsy and chronic headache. There
was no statistically significant difference between two groups in terms of frequency of co-morbid psychiatric disorder (p=0.967). However,
anxiety disorder is more frequent in the headache group whereas ADHD in epilepsy group.
