Amaç: Bu natüralistik izleme çalışması ile, şizofreni veya hezeyanlı bozukluk tanılı hastalarda haloperidol ve yeni antipsikotiklerin lipit metabolizması üzerine etkilerinin karşılaştırılması amaçlanmıştır. Yöntem: Çalışmaya en az bir haftadır (depo antipsikotik alanlarda bir aydır) ilaç kullanmayan 109 hasta ile başlanmış, haloperidol, klozapin, olanzapin, risperidon, ketiapin veya sülpirid kullanımı olan 64 hastanın verileri istatistiksel değerlendirmeye tabi tutulmuştur. Hastaların tedavi protokollerine müdahalede bulunulmamıştır. Çalışmanın başlangıcında ve 2 ay sonra, tedavisine uyan hastaların kan lipit değerleri ölçülmüş, BMI’leri hesaplanmıştır. Bulgular: Yeni antipsikotiklerden klozapin, olanzapin ve ketiapinin gruplarında HDL kolesterolün anlamlı derecede azaldığı ve haloperidol grubunda HDL kolesterol değerlerinde düşmeye eğilim olduğu gözlenmiştir. Yeni antipsikotik grubunda TG’lerin yükselme eğiliminde olduğu, ancak bu artışın istatistiksel anlamlılıkta olmadığı, haloperidol grubunda ise TG’lerde bir değişimin olmadığı bulunmuştur. Total kolesterol, VLDL ve LDL’yi olanzapin ve ketiapin yükseltmiş, ancak anlamlılığa ulaşmamıştır. BMI ne yeni, ne de klasik antipsikotik ilaç alanlarda anlamlı olarak değişmemiştir. Sonuç: Antipsikotikler ve özellikle ikinci kuşak antipsikotiklerle tedavi sırasında, dislipideminin sık olarak ortaya çıkabileceği ve bunların özellikle kardiyovasküler sistemi olumsuz etkileyeceği unutulmamalıdır. Obezite ve hiperlipidemi gibi risk etkenlerinin varlığında ilaç seçimi daha dikkatli yapılmalıdır. Tedavide bu grup ilaçlar seçilecekse ve risk etkenleri de varsa iyi monitörize edilmelidir.

Objective: In this naturalistic follow up study, the influence of haloperidol and new generation antipsychotics on lipid metabolism is aimed to compare in patients with schizophrenia or delusional disorder. Method: The participants were 109 patients who were drug free at least for one week (for depot antipsychotic users, drug free at least one [month]) at the beginning. The data of 64 patients who were receiving haloperidol, clozapine, olanzapine, risperidone, quetiapine or sulpiride were analyzed. No intervention was made to the treatment regimens of the patients. The plasma lipid values, body mass indexes of the patients were determined at the beginning and after the second [month] of the treatment who had complied with the treatment regimens. Results: In clozapine, olanzapine and quetiapine groups, the HDL-cholesterol level was found to be decreased significantly, and in haloperidol group, it was found to have a trend of decreasing. Triglyceride level was found in a trend of rising that was insignificant in the new antipsychotics receiving groups while there was no change in triglyceride level in haloperidol group. Total cholesterol, VLDL and LDL levels were found to have a trendy rising in olanzapine and quetiapine users. There was no significant change among body mass index both in new and old generation antipsychotics receivers. Conclusion: The occurrence of dyslipidemia and its negative effects to cardiovasculary system should be kept in mind while prescribing antipsychotics particularly second generation antipsychotics. In the presence of risk factors such as obesity and hyperlipidemia, the choice of drugs should be made more carefully. If these drugs have to be chosen for treatment, the patients have to be well monitorized
