Parkinson hastalığının seyrinde, çoğunluğu dopaminerjik tedavinin yan etkisi olarak %30 kadar psikoz olgusu ortaya çıkmakta, önemli yeti yitimine neden olmakta ve hastalığın tedavisinde baş edilmesi gereken en önemli sorunlardan birini oluşturmaktadır. Aripiprazol dopaminerjik sistem üzerine diğer antipsikotik ilaçlardan farklı bir etkiye sahip yeni bir atipik antipsikotik ilaç olup; dopamin D2 ve serotonin 5HT2A reseptörlerine kısmi agonist, serotonin 5HT1A reseptörüne antagonist etki eder ve dopamin iletim düzenleyicisi olarak tanımlanmıştır. Bu nedenle Parkinson ile ilişkili psikozun tedavisinde ümit verici bir seçenek olabileceği düşünülmüştür. Biz benzer bir olguda, 51 yaşında, erkek, Parkinson hastalığı (PH) olan, bu nedenle apomorfin kullanan ve psikoz tablosu gelişen bir hastada, 10 mg/gün aripirazol şeklinde tedavi uyguladık. Bu doz ile psikotik belirtilerde iyileşme sağlandığını ve iyilik halinin 4 hafta devam ettiğini gözlemledik. Ancak daha sonra psikotik belirtiler yeniden ortaya çıktı ve tedaviye devam edilemedi.
Literatürde PH ile ilişkili psikozda aripirazol kullanımı ile ilgili olgu bildirimlerine göz atıldığında; tüm bildirimlerdeki toplam 26 olgunun ancak 11 'inde olumlu sonuç alınabildiği, 15 olguda ise psikozun kötüleşmesi, PH'nın kötüleşmesi, hem psikozun hem PH'nın kötüleşmesi, etkisizlik veya ilacın tolere edilememesi gibi nedenlerle tedaviye devam edilemediği görülmektedir. Sonuç olarak, bu gruptaki hastalar için yeni bir seçenek olarak klinisyenlerin ümitle baktığı arip­iprazol, bizim olgumuzda bekleneni verememiştir.

Psychosis occurs in up to 30% of patients with Parkinson's disease (PD), predominantly as a side effect of dopaminergic treatment, cause a great disability and this situation is one of the most difficult challenges in the care of PD. Aripiprazole is a new atypical antipsychotic that is a partial agonist at the dopamine D2 and serotonin 5HT1A receptors and an antagonist at serotonin 5HT2A receptors with the different effect on dopaminerjic system from other antipsychotics and it is described as regulator of dopamine transmission. So it might be a promising approach for treatment of PD related psychosis. We used 10 mg aripiprazol daily on a 51 [year] old, male patient with PD, who had used apomorphine for PD and had psychosis at same time. An improvement with the psychotic symtoms at this dose of aripiprazole achieved and continued for 4 weeks. Then psychotic symtoms relapsed and he discontinuated treat­ment. There are case reports related to use of aripiprazole on the PD related psychosis, only 11 of 26 cases have a good outcome, while 15 cases had discontiuned their treatment due to worsening of PD, or psychosis, or worsening of both, lack of efficacy or intolarance. In conclusion, it seems aripiprazole which clinicians considered a promising choice, failed to fulfill the expectations.
