Amaç: Bu çalışmada, panik bozukluğu tanısı koyulan hastalarda çocukluk dönemi ve ilk atak öncesi
erişkin dönemdeki olumsuz yaşam olaylarının sıklığının saptanması ve bu sıklığın solunumsal
belirtili olan ve olmayan alt gruplar arasındaki karşılaştırılması amaçlanmıştır. Çalışmanın ikincil
amacı solunumsal ve solunumsal olmayan alt gruplar arasındaki ölçek puanları ve sosyodemografik
veriler açısından farklılığın araştırılmasıdır.
Yöntem: Gazi Üniversitesi Psikiyatri Polikliniğine başvuran ve halen ayaktan tedavisi devam eden,
DSM-IV tanı ölçütlerine göre panik Bozukluğu tanısı alan 85 hasta bu çalışmaya dahil edilmiştir.
Hastalara, Sosyodemografik ver formu ve görüşme formunun yanı sıra Yaşam Geçmişi Anketi, Rahe
ve Holmes sosyal olaylara uyumu ölçüm ölçeği, Panik Agorafobi Ölçeği, Beck Depresyon envanteri
uygulanmıştır. Hastalar solunumsal olan ve solunumsal olmayan tip olmak üzere gruplandırılmı
ş ve gruplar sosyo-demografik veriler, ölçek puanları gibi çeşitli değişkenler açısından karşılaştı-
rılmıştır.
Bulgular: Çalışma grubu, “solunumsal belirtili olan ve solunumsal olmayan panik bozukluğu” olmak
üzere 2 alt gruba ayrılmıştır. 59 hastanın [%69.4] solunumsal alt gruba, 26 hastanın ise [%30.6]
solunumsal olmayan alt gruba dâhil olduğu belirlendi. Gruplar arasında agorafobi ve beklenti anksiyetesi
açısından fark saptanmazken, noktürnal atak solunumsal alt grupta anlamlı olarak daha
fazla saptanmıştır Çalışmaya katılan hastaların %70.6’sında en az 1 çocukluk çağı ve %87.1’inde ilk
panik atak öncesi 1 yıl içinde en az 1 olumsuz yaşam olayı öyküsü tanımlanmıştır. Solunumsal olan
ve olmayan alt tipler arasında yaşam olayları sıklığı, Beck depresyon ve Panik Agorafobi ölçeği puanları
açısından anlamlı farklılık saptanmamıştır.
Tartışma ve Sonuç: Panik bozukluğun solunumsal ve solunumsal olmayan alt tipleri arasında
farklılık olabileceği yönünde literatürde az sayıda çalışmada çelişkili bulgular göze çarpmaktadır.
Bu çalışmada elde edilen bulgular solunumsal alt tipte, noktürnal atakların daha sık olduğunu göstermektedir.
Bu konuda daha geniş örneklemlerle yapılacak çalışmalara ihtiyaç vardır.

Purpose: Primary aim of this study is to determine the prevalence of childhood and recent negative
life events and compare their association with respiratory and nonrespiratory subgroups. Secondary
aim of the study is to compare the respiratory and nonrespiratory subtypes in terms of sociodemographic
variables and clinical scale scores.
Method: Eighty-five patients who attended to the Psychiatry department of Gazi University and diagnosed
as Panic disorder according to DSM-IV criteria were included in the study. Patients who accepted
to participate in the study were given were requested to complete sociodemographic data
form, Life History Questionnaire, Social Readjustment Rating Scale of Holmes and Rahe, Beck’s Depression
Inventory and Panic Agoraphobia Rating Scale. Later the patients were divided into two groups
as respiratory and non-respiratory subtypes and the two groups were compared with each other
in terms of the above variables.
Findings: Number of patients in respiratory subtype of patients with panic disorder and non-respiratory
subtype of panic disorder were 59 [69.4%] and 26 [30.6%] respectively. The groups were not
different from each other in terms of anticipatory anxiety and agoraphobia, whereas nocturnal attacks
were more common in respiratory subtype. At least one childhood negative life events were
present in 70.6% of the patients and 87.1% of the patients had at least one recent negative life event. Respiratory and non respiratory sub groups did not differed from each other in terms of negative
life events and scores of Beck’s Depression Inventory and Panic-Agoraphobia Rating Scale.
Conclusion and Discussion: Few studies investigate the differences between respiratory and nonrespiratory
subtypes of panic disorder and the results were inconsistent. Results of the present study
indicate that nocturnal attacks were more common in respiratory subtype in this sample. Studies
with larger samples are needed to clarify this point.
