Amaç: Şizofreni hastalarında antipsikotik tedaviye uyumsuzluk sık karşılaşılan bir sorundur. %80’e varan oranda hastaların ilaç kullanımının kesildiği bildirilmiştir. Tedaviye uyumsuzluk nedenlerinin belirlenmesi, uyumun iyileştirilmesi için çözüm olasılıklarını arttırabilir. Bu raporun amacı, gözlem çalışması sonuçlarına göre şizofreni ve şizoafektif hastalıkta antipsikotik tedavi uyumunu irdelemek ve klinik deneyimler ve literatür bilgileri ışığında olası çözümleri tartışmaktır.

Yöntem: Şizofreni veya şizoafektif hastalığı olan ve akut dönemde veya ayaktan tedavi gören 174 hastada antipsikotik tedavi uyumu anket çalışması gerçekleştirilmiş ve elde edilen sonuçlar uyumla ilgili literatür taraması ve güncel klinik deneyimlerin tartışılmasıyla zenginleştirilmiştir. Bulgular: Anket çalışması sonuçlarına göre hasta uyumsuzluğu uzun dönemde %25, akut dönemde ise %51 bulunmuştur. Her iki dönemde de uyumsuzluk sebeplerinin başında hastalığın yadsınması ve ilacın oluşturan etkileri gelmiştir.

Sonuçlar: Antipsikotik tedavide uyum hastalık semptomlarının hasta psikolojisi üzerindeki etkisinden damgalamaya, var olan ya da oluşması ndan korkulan ilaç yan etkilerinden ilaç kullanım zorluklarına kadar pek çok faktörle ilintilidir. Hastanın uyumu yan etkisi düşük, tolere edilebilirliği yüksek, basit doz aralıklarına sahip, kullanım rahatlığı sağlayan ilaçlarla artırılabilir

Aim: Antipsychotic drug non-compliance is a frequent problem in thetreatment of patient with schizophrenia. Up to 80% drugdiscontinuation rates were reported in clinical studies. Identifyingcauses of drug non-compliance may help clinicians to find solutionsfor improving compliance. The aim of this report is to examine thecompliance to antipsychotic treatment in schizophrenia andschizoaffective disorder according to the results of observationalstudy and to discuss the possible solutions by means of clinicalexperiences and literature information.Methods: 174 patients with schizophrenia or schizoaffective disorderreceiving treatment in acute or out-patient settings were interviewedfor compliance to antipsychotic treatment using a questionnaire.Results and possible solutions were discussed in the light of recentliterature and current clinical experiences. Results: Patient non-compliance was found 25% in long period and51% in acute period. For both periods, major causes for non-compliance were denial of disease and sedative effects of the drugs. Conclusion: Compliance to antipsychotic treatment is related withseveral factors like the effects of disease symptoms on psychology ofpatients, social stigma, side effects of the drug that are known orpossible to occur, and difficulties in using the drug. Compliance of thepatient can be increased by the drugs with few side effects, hightolerability, simple dosage range, and ease of use.
