Amaç: Bu çalışmada, vitiligo ve kronik ürtiker olmak üzere iki farklı cilt hastalığının kendilik saygısı üzerindeki
etkilerinin araştırılması amaçlanmıştır.
Yöntem: Çalışmaya İstanbul'un dört büyük hastanesinin Dermatoloji Poliklinikleri'ne başvuran, vitiligo ve
kronik ürtiker tanıları almış, 16-60 yaş arası 50?şer hasta alınarak, 50 sağlıklı kontrol grubu ile karşılaştırıldı.
SCID-I, Kendilik Saygısı Derecelendirme Ölçeği uygulandı., SPSS for Windows 10.0 istatistik paket programı
kullanıldı.
Bulgular: Yaş ortalaması vitiligoda 35.82±12.56, kronik ürtikerde 38.66±10.61 ve kontrol grubunda
35.98±12.49?dur. Vitiligo grubunun % 48'i , kronik ürtiker grubunun % 68'i ; kontrol grubunun % 50'si kadındır.
Kontrol grubunun kendilik saygısı toplam puanı, vitiligo ve ürtiker gruplarına göre anlamlı derecede
daha yüksektir. Ürtiker grubunda distimik bozukluk ve genelleşmiş anksiyete bozukluğu, vitiligo grubuna
göre anlamlı derecede daha fazladır. Gruplar arasında diğer SCID-I tanıları görülme sıklıkları bakımından
istatistiksel olarak anlamlı bir farklılık yoktur.
Tartışma: Çalışmamızda her iki cilt hastalığında kendilik saygısı, kontrol grubuna göre daha düşük olduğu,
ancak iki grup arasında anlamlı fark bulunmadığı, hasta gruplarında distimik bozukluk ve yaygın anksiyete
bozukluğu kontrol grubuna göre yüksek oranda olmasına rağmen, ürtiker grubunda vitiligo grubuna göre
anlamlı derecede daha fazla olduğu saptanmıştır. Her iki cilt hastalığı kendilik saygısında düşüşe neden
olurken, yaşanan psikiyatrik bozuklukların oranının farklı olduğu görülmektedir.

Objective: The relationship between skin diseases and psychological factors have been mentioned by may researchers. The aim of this study
was to investigate the impact of two different kinds of skin disorders upon self-esteem.
Method: The sample was composed of 50 vitiligo and chronic urticaria patients recruited from dermatology outpatient clinics of four major
hospitals in Istanbul. The age range of the sample was 16-60 [year]s. The control group was 50 healthy volunteers matched with age and education.
We applied the sociodemographical data form developed by us, The Turkish version of the Structured Clinical Interview for DSM-IV Mental
Disorders (SCID-I), Self-esteem Rating Scale (SERS). For statistical analysis SPSS for Windows 10.0 package programme was used.
Results: The mean age of vitiligo group was 35.82±12.56, chronic urticaria group was 38.66±10.61 and control group was 35.98±12.49.
48 % of the vitiligo group was female and 52 % was male. 68 % of chronic urticaria group was female and 32 % was male. 50 % of the control
group was female and 50 % was male. 58 % of the vitiligo group was married and 42 % was single. 70 % of the chronic urticaria group
was married and 30 % was single. 68 % of the control group was married and 32 % was single. 60 % of the vitiligo group graduated from
elementary school, 22 % from high school, 18 % from university. 70 % of the chronic urticaria group was from elementary school, 24 % from
high school and 16 % from university. The total scores of the self-esteem of the control group were significantly higher than vitiligo and chronic
urticaria group (p<0.001). The rates of dysthymic disorder and generalized anxiety disorder were found higher in the vitiligo and chronic
urticaria group than control group (p<0.05), but the other diagnoses in SCID-I were not statistically different between patients and control
group (p>0.05).
Discussion and Conclusion: In our study, we found that the scores of self-esteem in patients with two different kinds of skin disorders were
lower than control group (p<0.001), but there is no significant difference between the two patient groups. Although the rates of dysthymic
disorder and generalized anxiety disorder in patient groups were higher than the control group, the prevalence of dysthymic disorder and
generalized anxiety disorder was higher in chronic urticaria group than vitiligo group (p<0.05). While both of the skin disorders causes decrease
in self-esteem, the rates of the comorbid psychiatric disorders along with skin disorders were different. Our findings were discussed in
light of literature.
