Epileptik Çocuk ve Ergenlerde Psikiyatrik Başvuru ve Tedavi

Amaç: Epileptik çocuk ve ergenlerde psikiyatrik sorunlar sık
gözlenmektedir. Bu çalışmada epileptik çocuk ve ergenlerdeki
psikiyatrik sorunlar ile çocuk ve ergen psikiyatrisi polikliniklerine
başvuru arasındaki ilişkinin araştırılması amaçlanmıştır.
Yöntem: Bir üniversite hastanesi çocuk nörolojisi ve epilepsi
polikliniğine ardışık olarak başvuran 64 epileptik çocuk ve ergen
çalışmaya alınmıştır. Olgulara sosyodemografik veri formu ve 4-18
Yaş Çocuk ve Gençler için Davranış Değerlendirme Ölçeği (ÇGDÖ)
uygulanmıştır. Bütün olgularla tanısal psikiyatrik görüşme yapılmış ve
DSM-IV ölçütlerine dayalı olarak tanı konmuştur.
Bulgular: Olguların %48.4(n=31)’ünde psikiyatrik bir tanı olmasına
karşın çocuk ve ergen psikiyatrisi bölümlerine başvurma ve tedavi
edilme oranı %29.7(n=19)’dir. On dört olgunun (%21.9) psikiyatrik ilaç
tedavisi aldığı görülmüştür. Başvurusu olan ve olmayan olgular
karşılaştırıldığında, iki grup arasında ÇGDÖ’nin somatik sorunlar,
suça yönelik davranış, saldırgan davranışlar, sosyal, düşünce ve
dikkat sorunları ile dışa yönelim, içe yönelim ve toplam alt ölçek
puanları arasında anlamlı farklılık bulunmuştur. Çocuk ve ergen
psikiyatrisi bölümlerine başvurusu olan olgulara psikiyatrik bozukluk
tanısı daha sık konulmuştur. Başvuruda bulunan ailelerin gelir düzeyi
daha yüksektir. Dikkat eksikliği sorunları klinik başvuruyu yordayan en
önemli etmen olarak saptanmıştır.
Sonuç: Epileptik çocuk ve ergenlerdeki yetersiz psikiyatrik hizmet alma
oranının yüksek olması, bu hastaların uygun tedavi edilebilmeleri için
çocuk nörolojisi ve epilepsi bölümleri ile çocuk ve ergen psikiyatrisi
bölümleri arasında daha fazla konsültasyon liyezon çalışması
yapılmasına gereksinim olduğunu ortaya koymaktadır.
Psychiatric referral and treatment in children and adolescents with epilepsy
Objective: Psychiatric problems are frequently observed in children
and adolescents with epilepsy. The aim of the present study was to
investigate the relation between psychiatric problems and mental
health service in children and adolescents with epilepsy.
Method: Sixty four children and adolescents with epilepsy who were
consecutively referred to the child neurology department and the
epilepsy unit of a university hospital were included in the study.
Sociodemographics questionnaire and Child Behavior Checklist
(CBCL) were applied to all of the cases. Then each cases were
interviewed and diagnosed according to DSM-IV criteria.
Results: Although 48.4% (n=31) of the cases have a psychiatric
diagnosis, the rate of cases who seek for psychiatric treatment was
only %29.7 (n=19). Fourteen cases (21.9%) were taking psychiatric
medication. When we compared the cases who referred to child
psychiatry departments and those who did not, there were differences
of the scores of some subscales between the two groups such as
somatic complaints, delinquent behavior, aggressive behavior, social,
thought, attention problems, externalizing, internalizing and total
problem subscales of the CBCL. Psychiatric diagnoses were more
frequent in children who referred to a child psychiatry department.
Family income level was found to be associated with the demand of
mental health service. Attentional problems were the most important
predictor for psychiatric referral.
Conclusion: High rate of unmet mental health service demand in
epileptic children and adolescents underscores the need for more
consultation-liaison studies among child psychiatry departments and
child neurology and epilepsy clinics in order to provide mental health
service to these patients.
