Alkolün santral sinir sisteminde yaygın etkileri olduğu uzun zamandan beri bilinmektedir. Son yıllarda nörogörüntüleme tekniklerinde görülen hızlı gelişim, alkolün beyin üzerindeki morfolojik ve nörofizyolojik etkilerinin daha ayrıntılı olarak çalışılmasına olanak tanımaktadır. 1970’li yıllardan itibaren bilgisayarlı tomografi ile başlayan nörogörüntüleme çalışmalarında, özellikle frontal lobların alkolün toksik etkisine daha duyarlı olduğu gösterilmiştir. Manyetik rezonans çalışmaları, önceki bilgisayarlı tomografi sonuçlarını desteklemek dışında, beyaz cevher ile subkortikal yapıların daha iyi çalışılmasını sağlamıştır. Sonraki nörofizyolojik çalışmalarda ise, alkol bağımlılarında ortaya çıkan frontal bozuklukları dengelemek üzere beyin sistemlerinin işlevsel düzeyde yeniden organize olduğu gösterilmiştir. Bu etkilerin ayıklık ile tersinir olduğu hem yapısal hem de işlevsel çalışmalarda tutarlı olarak yinelenen bir bulgudur. Son teknikler ile bu tersinirliğin beyinde aktif bir nörorejenerasyon sürecine işaret etmesi, hastalarda tedavi motivasyonunu artırmak açısından önemli görünmektedir

It is now well-known that alcohol causes widespread effects on central nervous system. Recent improvements in neuroimaging technology have significantly contributed to our understanding of the morphological and neurophysiologic effects of alcohol. Starting with the computerized tomography studies in the 1970’s, it is now well established that particularly frontal lobes are more sensitive to the toxic effects of alcohol. Magnetic resonance technique allows a more detailed investigation of the white matter abnormalities and subcortical structures in alcoholism. Recent neurophysiologic studies show that there is a compensatory reorganization of the brain system in alcoholics which balance the frontal lobe dysfunctions. The alcoholic brain damage is consistently shown to be reversible in both structural and functional studies and latest techniques point out an active regeneration process in alcoholic brain damage which seems to be an important reason to encourage patients for treatment.
