Huzurevi Yaşayanlarında Depresif, Psikotik ve Bilişsel Değişiklikler

Huzurevinde yaşayan 64 kişide depresif, bilişsel ve psikotik değişiklikler araştırıldı. Depresif belirtilerin değerlendirilmesinde Hamilton Depresyon Değerlendirme Ölçeği (HDDÖ), bilişsel değişikliklerin değerlendirilmesinde Kısa Akıl Muayenesi (KAM) ölçeği ve psikotik semptomların değerlendirilmesinde Kısa Psikiyatrik Değerlendirme Ölçeği (KPDÖ) kullanıldı. Olgular iki gruba ayrıldı: Birinci gruba KAM ölçeğinden 23 ve daha düşük puanı olanlar, ikinci gruba KAM ölçeğinden 24 ve daha fazla puanı olanlar alındı. Birinci grupta yaş ve HDDÖ puanı ortalamaları ikinci gruptan anlamlı derecede yüksek, KAM puanı ortalamaları ise anlamlı derecede düşük bulundu (sırası ile t= 2.33, t= 2.84, t= 10.6). Ayrıca bilişsel durum ile; depresif, psikotik belirti puanlan, yaş, huzurevinde kalış süresi arasında ilişki bulundu (sırası ile r= -0.263, F= 4.6, r= -0.46, F= 16.9; r= -0.24, F= 3.74; r= 0.48, F= 18.9).
Deneklerin cinsiyet değerlendirilmelerinde KAM puanı erkeklerde anlamlı derecede yüksek, KPDÖ puanı ve HDDÖ puanı anlamlı derecede düşük bulundu (sırası ile t= -2.74; t= 2.59; t= 2.36). Biz bu sonuçlan yaşlıda demans belirtileri ile depresif belirtilerin birbirinden aynlamayacak kadar benzer olduğu, ya da yaşlıda depresif ve psikotik belirtilerin beyin yıkımı sonucu olarak ortaya çıktığı ve gençlerdeki benzer belirtilerle farklı psikopatolojik anlamlan olduğu şeklinde yorumladık.
Depressive Psychiatric And Cognitive Changes In Individuals Who Lives In a Rest Home
Depressive, psychotic and cognitive changes were investigated in 64 individuals who lives in a rest home. Depressive symptoms were quantified with the Hamilton Depressive Rating Scale (HDRS). Cognitive dysfunction was assessed with the Mini-Mental State Examination (MMSE), and psychotic symptoms were rated with the Brief Psychiatric Rating Scale (BPRS). Subjects who took 23 and lower scores were included in the first group according to MMSE scale. Subjects who took 24 and more scores were included in the second group according to MMSE scale. In the first group, age and HDRS scores were found to be significantly increased and MMSE scores were found to be significantly decreased as compared to second group (t= 2.33; t= 10.6; t= 2.84, respectively). Also cognitive status were correlated to depressive, psychotic symptom scale scores age and staying time. In addition to these psychotic and depressive symptom scale scores were correlated with each other (r= -0.263, F= 4.6, r= -0.46, F= 16.9; r= -0.24, F= 3.74; r= 0.48, F= 18.9 respectively). When the groups were compared according to sex, MMSE scores were higher and BPRS scores were lower in the male subjects than in the female subjects. Finally, these results show that there is some ovarlap between symptoms of depression and cognition among the elderly. In addition to these, in elderly individuals depressive, psychotic and cognitive symptoms become as a result of brain defoliation. These symptoms have a different meaningful of psychopathologic changes from the same symptoms that are seen in younger individuals.
