Çocuk psikiyatrisi polikliniklerine en sık başvuru nedenlerinden biri olan enkoprezisin erkek hastalarda daha sık görüldüğü, ergenlik döneminde de devam edebildiği ve pek çok psikiyatrik hastalıkla binişik bulunabildiği bilinmektedir. Enkopreziste hastalığın sürmesine neden olan bilinç dışı süreçler ve psikososyal etkenler değerlendirildiğinde elde edilen veriler tedavi sürecine önemli düzeyde katkı sağlamaktadır. Bu olgu sunumunda ikincil enkoprezisi olan bir ergen hastanın psikodinamik yaklaşımla değerlendirilmesi ve takip süreci ele alınmıştır. Bu olgu aracılığıyla enkoprezis ile başvuran hastaların giderek süregen hale gelen belirtilerini ve tedaviye karşı oluşan dirençlerini anlamamıza yarayan psikodinamik bağlantıların önemine dikkat çekilmesi amaçlanmıştır.

Encopresis, among most common complaints within presentations to child and adolescent psychiatry units is much more common in male patients, might proceed into adolescence, and be comorbid with many psychiatric disorders. Evaluating psychosocial factors and unconscious processes that might lead to persistence of the disorder might create a signi? cant positive in? uence on thetreatment process. In this case report, we aimed to discuss the psychodynamic evaluation as well as approach and follow-up process of an adolescent with secondary encopresis. Through this case presentation, it is intented to drive attention to the importance of psychodynamic connections that help understanding patients with persistent encopresis and treatment resistance.
