Bir Olgu Dolayısıyla Fronto-Temporal Demansın Psikiyatrik İzdüşümü
Frontal lobların lokalize dejenerasyonu ile seyreden fronto-temporal demans, Alzheimer hastalığından
sonra en sık görülen primer dejeneratif demans tipidir. Hastalığın klinik bulguları oldukça heterojendir.
Başlıca iki klinik tipten sözedilebilir: Esas olarak kişilik değişikliği ile karakterize frontal tip ve ilerleyici lisan
bozukluğu ile karakterize temporal tip.
Frontal tipte semptomlar geniş bir spektrumda dağılım gösterirler. Ancak hastalığın klinik gösterimi ve
baskın olan semptomlar sosyal davranım ve kişilik değişikliğidir. Frontotemporal demansta hastalığın erken
dönemlerinde psikopatolojik tabloları taklit ediyor olması tanıda gecikmelere yol açmaktadır. Bu yazıda
mevcut nörolojik defisitlerine göre daha fazla işlev kaybı gösterdiği ve ikincil kazançların hakim olduğu
düşünülen bir olguda saptanan frontotemporal demansın özellikleri sunulmuştur. Olgu fizik tedavi ve
rehabilitasyon kliniği tarafından tedaviye uyumsuz olması sebebiyle psikiyatriye yönlendirilmiştir.Yapılan
detaylı nöropsikolojik değerlendirme ve BBT bulguları sonucunda hastaya fronto-temporal demans
tanısı konmuştur. Olgu, bu tür hastaların sıklıkla yanlış ve/veya eksik tanılarla izlenmesi sonucunda yanlış
yönlendirilmesi ve tedavide işbirliğinin bozulması gibi sorunların tartışılması olanağını sağladığı için dikkate
değer olduğu düşünülmektedir.

Frontotemporal dementia (FTD) is the most common form of primary degenerative dementia after Alzheimer’s disease. The clinical findings of
the disease are heterogeneous. There are mainly two clinical presentations: Frontal variant characterized by progressive personality change
and dysexecutive function and temporal variant characterized by progressive [language] disorder. The most common behavioral manifestation
of FTD is an early change in personal and social conduct characterized by difficulty in modulating behavior to the social demands of a
situation. Because patients with FTD have different behavioral manifestations early in the disease, they may initially be confused with patients
having primary psychopathological disorders. In this paper we present a case with frontotemporal dementia who was referred to psychiatry
because of his incompliance with the treatment in the physical therapy and rehabilitation clinic. The patient was thought to get advantage of
his situation because of his secondary gains, because the degree of his social deficit was outweighing his neurological pathology. Following
the examination of the patient with neuropsychological tests and computed tomography, the patient was diagnosed to have frontotemporal
dementia. This case was considered to be noteworthy because it emphasizes the fact that such patients may sometimes be misdirected by
clinicians because of a deficient diagnosis.
