Giriş: Kronik hastalığın medikal tedavisinin psikiyatrik bozukluklara etkisi konusunda literatürde farklı görüşler vardır. Çalışmamızda, subklinik hipotiroidi hastalarında anksiyete ve depresyon düzeylerini ve tiroksin yerine koyma tedavisinin bu konuda etkinliğini araştırdık Materyal ve Metod: Yeni tanı konmuş subklinik hipotiroidili hasta ile benzer özelliklere sahip ötiroid kontrol grubu çalışmanın örneklemini oluşturdu. Katılımcılara Beck Anksiyete ve Beck Depresyon Ölçekleri uygulandı. Hasta gruba Levotiroksin tedavisi düzenlendi. 6 hafta sonra tiroid hormonlarına tekrar bakıldı. Ötiroid duruma gelen hasta gruba aynı dozda Levotiroksin tedavisine 3 ay devam edildi. Tedavi sonunda aynı psikolojik ölçekler tekrarlandı. Sosyodemografik özellikleri ile birlikte tedavi öncesi ve sonrası elde edilen sonuçlar, hazırlanan hasta takip formuna kaydedildi. Bulgular: Hastaların 66'sı kadın, 8'i erkek iken, kontrol grubunun 64 kadın ve 8'i erkekti. Yaş ortalaması hastalarda 40,87±10,80 yıl ve kontrol grubunda ise 41,61±10,33 yıl idi. Kadın hastalarda, kontrol grubuna göre hem anksiyete hem de depresyon skorları yönünden anlamlı bir yükseklik vardı (p<0,05). Erkeklerde ise depresyon skorlarında hasta grupta, sağlam gruba göre anlamlı bir yükseklik bulunmuşken (p<0,05) anksiyete skorları açısından fark bulunmadı (p=0,188). Tiroid yerine koyma tedavisinin cinsiyetlere göre karşılaştırılmasında kadın hastalarda, tedavi sonrasında hem anksiyete hem de depresyon skorlarında anlamlı bir düşüş gözlendi (p<0,05). Erkeklerde ise istatistiksel bir fark tespit edilmedi (sırasıyla p=0,68 ve p=0,31). Sonuç: Hashimoto tiroiditine bağlı subklinik hipotiroidinin depresyon ve anksiyete gelişimi için risk faktörü olabilir. Özellikle subklinik hipotiroidili hastalarda tiroksin yerine koyma tedavisi, beck anksiyete ve depresyon skorlarında azalma sağlayabilir.

Objective: In our study, we investigated the effectiveness
of thyroxin replacement therapy on the levels of
anxiety and depression in patients with subclinical
hypothyroidisim. Method: Patients with new onset subclinical
hypothyroidism and euthyroid control group
constituded the subjects of our study. Beck Anxiety and
Beck Depression Scales were applied to these participants
in the beginning of the study. Patient group was
put on Levothyroxin therapy. Thyroid hormone levels
were checked before and six weeks after the treatment
was started. At the end of the study same psychologic
scales were repeated. Results: In the study group, 66
patients were female and 8 patients were male whereas
there were 64 female and 8 male subjects in the control
group. Average age was 40.87±10.80 [year] in the study
group and 41.61±10.33 [year] in the control group. In
female group, there was a significant elevation (p<0.05)
in both anxiety and depression scores compared to control
group. While there was a significant elevation in
depression scores of male patients compared to control
group, no difference was found in anxiety scores
(p=0.188). In female group comparison of thyroid
replacement therapy according to gender, a significant
decrease was observed in both anxiety and depression
scores after therapy (p<0.05). There wasn't any statistical
difference in male group (respectively p=0.68 and
p=0.31). Conclusion: Subclinic hypothyroidism related
to Hashimoto thyroiditis may be a risk factor for anxiety
and depression. Particularly thyroxin replacement therapy
in subclinical hypothyroidism may reduce the beck
anxiety and depression scores.
