Tianeptin, dibenzotiazepin türünde atipik bir trisiklik antidepresandır. Biyokimyasal olarak, tiyaneptinin tek ya da yineleyen dozlarda verildikten sonra, beyinde ve trombositlerde serotoninin presinaptik geri alınımını artırdığı gösterilmiştir. Tianeptin strese verilen hipotalamo-pituiter-adrenal eksen yanıtını azaltır ve stresin yol açtığı davranışsal sorunları düzenler. Farmakoterapötik bakımdan uyarıcı ya da sedatif değildir. Antidepresanlara karşı bağımlılık gelişimesi tartışmalı ve literatürde az değinilmiş bir konudur. Sunduğumuz bu olguda yüksek dozlarda tianeptin kullanımı olan bir hastanın tolerans durumu, yaşadığı yoksunluk süreçleri ve tedavisi ele alınmıştır.

Tianeptine is a dibenzothiazepine type an atypical, tricyclic antidepressant. Biochemically it has been shown that after single or repeated administration, tianeptine increases the presinaptic reuptake of serotonine at the brain and trombocytes. Tianeptine reduces the hypothalamic-pituitary-adrenal response to stress and antogonizes the behavoiral problems induced by stress. Its pharmacotherapotic effects are neither stimulant nor sedative. Addiction to antidepressant therapy is controversial and rarely defined at the literature. Here we represent the tolerance, withdrawal symptoms and treatment issues of the case whom misused tianeptine at extremly high doses.
