Bipolar I Bozukluk Hastaları Şizofreni Hastalarına Göre Diyabet ve Dislipidemi Ek Tanıları Açısından Farklı mı?

Amaç: Şizofreni tanısı almış hastalarda diabetes mellitus ve dislipidemi ek tanısını araştıran çalışmalar sık yapılmıştır. Bipolar I bozuklukta benzer çalışmalar şizofreniye kıyasla daha az ilgi görmüştür. Ayrıca bu iki gruptaki tıbbi ek tanıları karşılaştıran çalışmalar da azdır. Bu çalışma ile yatırılarak tedavi gören şizofreni ve Bipolar I bozukluk hastalarında glikoz ve lipit metabolizmalarının karşılaştırılması amaçlanmıştır. Yöntem: Bakırköy Ruh Sağlığı ve Sinir Hastalıkları Eğitim ve Araştırma Hastanesinde 2006 Haziran ve 2007 Mayıs tarihleri arasında en az iki kez yatırılarak tedavi görmüş olan 253 şizofreni ve 339 Bipolar I bozukluk tanılı hastanın kayıtları incelenmiş ve iki grup diabetes mellitus ve dislipidemi yönünden karşılaştırılmıştır. Bulgular: Bipolar I bozukluk hasta grubunda daha fazla oranda diabetes mellitus, bozulmuş açlık glikozu, yüksek trigiserid, düşük HDL-kolesterol düzeyleri saptanırken, şizofreni hasta grubunda daha fazla oranda yüksek total kolesterol ve LDL-kolesterol düzeyleri saptanmıştır. Ancak her iki grup arasındaki farklılıklar istatistiksel anlamlılığa varmamıştır. Sonuç: Bipolar I bozukluğu olan hastalarda, diabetes mellitus ve dislipidemi
Are Bipolar I Patients Different from Schizophrenic Patients in Terms of Diabetes and Dyslipidemia Comorbidity?
Objectives: The prevalence of type II diabetes mellitus in
patients with schizophrenia has been reported to be two
or four times greater than general population. Besides,
schizophrenia and elevated prevalence of diabetes mellitus
has been reported in several other psychiatric disorders
such as schizoaffective disorder, bipolar disorder
and major depression. There are many studies about
comorbidity of diabetes and dyslipidemia in schizophrenic
patients compared to bipolar patients. However,
there are only few studies releated with diabetes and
dyslipidemia comorbidity in both groups. In this study,
our aim was to compare the glucose and lipid
methabolism in schizophrenic and bipolar patients who
were hospitalized. Method: 253 schizophrenic patients
and 339 bipolar I patients who have been hospitalized
at least twice between June 2006 and May 2007 were
evaluated retrospectively. The diagnosis of diabetes and
dyslipidemia were compared. Results: Higher diabetes
and impaired fasting glucose prevalance, higher trigliserids,
lower HDL levels were found in bipolar group; total
cholesterol and LDL levels were higher in patients with
schizophrenia. But there was no statistical significance
between groups. Conclusion: The medical risk, because
of diabetes mellitus and dyslipidemia comorbidity, is
similar in bipolar patients compared to schizophrenic
patients. New studies are needed for the diagnosis of
diabetes mellitus and dyslipidemia. The results of studies
should be considered while choosing the psychiatric
medication.
