Amaç: Bolu’da toplum temelli ruh sağlığı hizmet modeline geçişte örnek teşkil etmek üzere, Bolu İzzet Baysal Ruh Sağlığı ve Hastalıkları Hastanesi’ne bağlı bir birim olarak hastaneden ayrı, mahalle içinde toplum ruh sağlığı merkezi ilk kez Mayıs 2008’de açılarak proje ile ilgili uygulamalara başlanmıştır.

Yöntem: Toplum temelli psikiyatri modelinde 150-200 bin nüfusluk coğrafi bölgelere birer toplum ruh sağlığı merkezi öngörülmesi, Bolu’nun ilçeleri ile beraber 250.000 nüfuslu il olması, aile hekimliği sisteminin de uygulanıyor olması, Bolu’nun proje için uygun bir seçim olmasını sağladı. Toplum ruh sağlığı merkezi olarak şehir merkezinde mahalle içinde sağlık ocağından boşalan aile hekimliğinin alt katı temin edildi. Hedef olarak öncelikle Bolu ilinde oturan psikoz (ICD-10 ölçütlerine göre F20-29) tanısı konan hastaların merkeze kaydedilmeleri planlandı. Aylık ruh sağlığı hastanesinde yatan veya ayaktan kayıtlı psikotik hasta bilgileri merkeze iletildi. Toplum ruh sağlığı merkezinde başta bir konsültan psikiyatrist olmak üzere, bir psikolog, üç hemşire, bir hizmetli, bir özel güvenlik görevlisi hastane kadrosundan görev-lendirildi. Toplum sağlığı merkezinden bir hemşire aile hekimleri ile koordinasyonu sağlamak, halk eğitim merkezi tarafından da resim ve el sanatları alanında iki öğretmen uğraşı tedavi-lerini yapmak üzere görevlendirildi. Haftanın belli günleri hasta ev ziyaretlerini yapmak üzere bir hemşire, bir psikolog, bir sağlık memuru ve bir şoförden oluşan gezici ekip oluşturuldu. Gezici ekip özel formu geliştirilerek ziyaretler imza ile tutanak altına alındı. Merkezde haftada bir gün yapılan poliklinikte hastaneden yönlendirilen, telefonla aranan veya gezici ekiple evde ziyaret edilerek çağrılan hastaların ayırıcı tanıları yapıldıktan sonra, tedavileri düzenlenerek merkeze kayıtları yapılmakta; tedavilerinin izlenmesinin yanı sıra psiko-eğitim, aile eğitimi ve sosyal beceri eğitimleri ile uğraşı tedavileri de rehabilitasyon çalışmaları olarak uygulanmaktadır. Hastaların belediye tarafından sağlanan servis ile merkeze transferleri sağlandı.

Bulgular: Bolu ilinde bir yıl içinde 496 psikotik hasta kaydına ulaşılmıştır. Projede ilk etapta Bolu merkez nüfus hedeflendiğinden, merkezdeki 253 psikoz hastasından 141’i toplum ruh sağlığı merkezine kaydedilmiş olup; 141 hastanın bir yıllık takipleri sonrası toplam hastaneye yatış sayılarının yarıya düştüğü saptanmıştır. Gezici ekip oluşturulduktan sonraki yedi ayda ise, 189 ev ziyareti yapılmış olup ulaşılan hastalardan yaklaşık %60’ının toplum ruh sağlığı merkezine gelmeyi kabul ettiği belirlenmiştir.

Tartışma: Toplum ruh sağlığı merkezi projesi bize, düzenli takip altındaki psikotik hastaların hastaneye mükerrer yatış sayılarını azalttığını göstermiştir. Bu proje kapsamında Bolu ilindeki tüm psikotik hastalara ulaşmayı ve merkezde kayıt altına almayı planlamaktayız.

Objective: In order to be a model for changing to community based mental health system in Turkey, we opened the first commnity mental health center in Bolu as a supplement center but in a district out of Bolu Izzet Baysal Mental Health Hospital on May 2008 and started to work as a project.

Methods: As Bolu has a population of 250.000 and again has a working family doctors system so Bolu was the suitable city for the project. We opened the center instead of primary health care, downstairs of family doctors office. First, we planned to record the psychotic patients with diagnozis F20-29 (as ICD-10 criteria) who live in Bolu. We took the psychotic patients records from Bolu Mental Health Hospital every [month]. The team of the center consists of one consultant pscychiatrist, one psychologist, three nurses, one cleaner, and one special security charged person (from hospital’s staff). One nurse was charged from community center to make the coordination between the family doctors and the center. Two teachers were charged by public education center. We formed a mobil-team with two nurses, one psychologist and one driver by using hospital’s car for visiting the patient’s at home. We also formed a new type of record for mobil-team in order to take the visits under signature. After the psychiatrist’s mental examination the registration of the patients who were called by phone, by mobil team or from hospital are made. The patients are taken by a service which is supported by municipality. In the center psycho-education, family education and social-skills training and lessons about painting and hand-arts are given as rehabilitation programmes.

Findings: After one [year] we reached to 496 psychotic patients records that live in Bolu. In our project we took city center’s patients (253) at first. Among these patients, 141 patients are registrated to the center. During one [year] period the total number of hospitalization of these patients, decreased to half. During the seven [month]s of mobil-team’s home visits, 189 home-visits were made. Sixty percent of these patients accepted coming to center.

Discussion: Bolu community mental health center project practices showed us that by following systematicly the treatments and rehabilitation of psychotic outpatients in community decreases the patients’ recurrent hospitalizations. We plan to reach all psychotics in Bolu and registrate them to the center.
