Frontotemporal demans (FTD) yavaş ilerleyen, başlangıç evresinde bellekten çok dil işlevlerinde bozulma ve/veya davranışsal sorunlarla giden ve sıklıkla parkinsonizmin de tabloya eşlik edebileceği frontotemporal lobar dejenerasyon (FTLD) alt tiplerinden biridir. Bu olguda erken yaşta davranış değişikliği ve depresif duygudurum nedeni ile antidepresan kullanan ve tedaviden fayda görmeyen hastanın FTD yönünden değerlendirilmesi uygun görüldü. Otuz altı yaşın-daki erkek hasta kliniğimize yakınları tarafından daha önce olmayan çocuksu davranışlar, çevre kayıtsızlık, iş görememe, kişilerarası ilişkiler sorun yakınmaları ile getirildi. Bu yakınmaları altı ay önce başlayan hasta, depresyon tanısıyla iki ay klomipramin ve maprotilin kullanmış, fayda görmemiş. Soygeçmişinde özellik bulunmayan hastanın kliniğimizdeki psikiyatrik muayenesinde, kendine bakımda azalma, dikkat, konsantrasyon ve bellekte azalma, çağrışımlarda dağınıklık, kendiliğinden sürdürülemeyen konuşma ve perseverasyonu vardı. Soyut düşüncede benzerlik ve farklılıkları bulamayan hasta atasözlerinin anlamını soyut düzeyde açıklayamadı. Saat çizme testinden 4/4 puan alan, mini mental eği-timsizler için yapılan testte 23 ve SET testinden 26 puan aldığı gözlendi. Duygulanımda azalma ve abulisi olan hastaya yattığında olanzapin 10 mg/gün ve memantin 5 mg/gün başlandı. Memantin dozu kademeli olarak artırılarak 20 mg/güne çıkıldı. Sonrasında hastanın depresif duygudurumu nedeniyle reboksetin 4 mg/gün başlanıp, dozu kademeli olarak artırılarak 8 mg/gün'e çıkıldı. Tedavisinin üçüncü haftasında kısmi düzelme gözlendi. Biz bu olguda, hastanın belirtilerinin depresyonla karışabileceği, genç yaş ve ön planda davranış değişiklikleri olması durumunda ayırıcı tanıda FTD'nin göz önünde bulundurulması gerektiğini tartışmak istedik.

Frontotemporal dementia (FTD) is one of fronto-temporal lobar degeneration subtypes which
progresses gradually and reveals with the impairment in the [language] function in the initial stages and/or with behavioral problems rather than in memory and often accompanies with parkinsonism. In this case, the patient who got antidepressant treatment because of dramatically behavioral change for his age and depressive mood could not benefit from the treatment and then FTD evaluation was appropriate. His relatives and acquaintances to our clinic brought 36 [year]s-old male patient because of his infantile behaviors, indifference to the environment, not able to work and lastly complaints on interper-sonal relationships. The patient, whose complaints have been started six [month]s ago, used clomipramine and maprotiline along two [month]s with the diagnosis of depression. However, he could not benefit from this treatment. In the psychiatric examination with our patient without a significant family medical history, decrement in his self-care, attention, concentration, memory, disorganized association, decrement in spontaneous speech, and also perseveration were found. Our patient who was not able to find the similarities and differences at the abstract level also could not explain the meaning of sayings in Turkish. It was observed that he got 4 points out of 4 total scores on the clock drawing test, 23 points from mini mental test for illiterates and 26 points on the SET test. Olanzapine 10 mg/day, memantine 5 mg/day were given to the patient for the reduction in his affect and abuly. Doses of memantine were gradually increased to 20 mg/day. Because of his depressive mood, reboxetine 4 mg/day was given and increased gradually to 8 mg/day. Partial recovery was observed in the third week of treatment. In this case, it is desired to discuss that FTD should be considered for diagnosis because of the similar symptoms that can be easily confused with symptoms of depression and when patients are young as well as dramatically, behavioral changes do exist in the first place.
