Amaç: Şizofreni hastaları ve yakınlarının yaşam kalitesini yordayan etkenlerin ve zaman içindeki değişimlerinin belirlenmesi amaçlanmıştır. Yöntem: Şizofreni tanısıyla ayaktan izlenen 75 hasta psikopatoloji, içgörü, duygu dışavurumu, aile ortamı ve yaşam kalitesi; birer yakınları ise duygu dışavurumu, aile ortamı ve yaşam kalitesi açısından değerlendirildi. Altı ay sonra 32 hasta ve yakınlarında aynı değerlendirmeler yinelendi. Bulgular: İki değerlendirme arasında hastaların içgörü düzeyinde anlamlı artış olmuş; pozitif belirtiler ve depresyon azalırken (p<0.05), hasta ve yakınlarının yaşam kalitesinde anlamlı deği-şiklik olmamıştır. İçgörü azlığı, benlik saygısı düşüklüğü, depresyonun şiddetli ve aile içi çatışmanın fazla olması hastalarda yaşam kalitesinin azalmasını yordamaktadır. İkinci değerlendirmelerde içgörü azlığının yordayıcı etkisi sürerken, aile içi hoşgörünün az, beklentilerin yüksek olması ve ilaca bağlı hareket bozuklukları yaşam kalitesini olumsuz etkilemektedir. Hasta yakınının yaşam kalitesini pozitif, negatif ve depresif belirtiler ile aile ortamındaki çatışma ve kuralcılık yordamaktadır. Bu bağlantı ikinci ölçümlerde de sürmektedir. Literatüre göre aile ortamı ve duygu dışavurumu ile hastalık belirtilerinin şiddeti hastalığın gidişiyle ilgili önemli yordayıcılardır. Yaşam kalitesinin belirti şiddeti, ilaca bağlı etkiler ve içgörü ile ilişkisilidir. Sonuç: Hasta yakınlarının yaşam kalitesinin hastalığa ait belirtilerden etkilenmesi hasta ve yakınlarına yönelik psikoeğitimsel girişimlerin uygulanacağı çoklu aile gruplarının önemine işaret etmektedir

Objective: The aim of this study was identifying the factors that predict the quality of life (QOL) of schizophrenia patients and their relatives, and the change in the predictors in time. Methods: Seventy-five outpatients diagnosed as schizophrenia were assessed for psychopathology, insight, expressed emotion, family environment and quality of life while one of their relatives were assessed for expressed emotion, family environment and QOL. The same assessments were repeated in 32 of the patients and their relatives, six [month]s later. Results: The insight levels of the patients showed significant increase between two assessments; while positive symptoms and depression decreased (p<0.05), no significant change was detected in patients or their relatives. Lack of insight, low self-esteem, severity of depression and severe family conflict were found to be predictors of low quality of life. In the second assessments, while the predicting power of lack of insight was still significant, low tolerance and high expectation in the family environment, and drug-related movement problems negatively affected the QOL. The relatives? quality of life was predicted by positive, negative and depressive symptoms, as well as conflict and rigidity in the family environ-ment. These relationships persisted in the second assessments as well. According to the literature, family environment, expressed emotion and severity of the symptoms are among the important predictors of the prognosis. Relationships are also reported between QOL and severity of symptoms, adverse drug reactions and insight. Conclusion: The effect of patients? symptoms on the relatives? QOL indicates the importance of multiple family therapy groups where psycho education techniques would be used on both the patients and their relatives.
