Amaç: Bu çalışmanın amacı panik bozukluğu ve yaygın anksiyete bozukluğu olan hastalarda essitaloprama kortizol, dehidroepiandrosteron-sülfat ve prolaktin yanıtlarının ölçülmesi suretiyle serotonerjik sistem etkinliğini incelemek ve bu sistemin panik bozukluğu ve yaygın anksiyete bozukluğunun patofizyolojisindeki rolünü araştırmaktır. Yöntem: Panik bozukluğu (n=24) ile yaygın anksiyete bozukluğu (n=21) olan hastalarda ve sağlıklı kontrol (n=20 ) deneklerinde bazal ve essitalopram uygulaması sonrasında kortizol, dehidroepiandrosteron-sülfat ve prolaktin değerleri ölçüldü. Bulgular: Panik bozukluğu ve yaygın anksiyete bozukluğu gruplarında essitalopram uygulamasına hormonal yanıt kontrol grubuna benzerdi. Panik bozukluğu ve yaygın anksiyete bozukluğu olan hastalarda bazal dehidroepiandrosteron-sülfat seviyesi kontrol grubuna göre düşüktü. Hem kontrol hem de hasta gruplarında bazal dehidroepiandrosteron-sülfat seviyesinin kadınlarda erkeklerinkinden düşük olduğu bulundu. Sonuç: Hastalarda ve kontrol grubunda essitalopram uygulamasına anlamlı yanıt bulunamadı. Bazal dehidroepiandrosteron-sülfat seviyesinin panik bozukluğu ve yaygın anksiyete bozukluğu olan hastalarda düşük oluşu, bu hormonun bu hastalıkların patofizyolojisinde bir rol oynuyor olabileceğini düşündürmektedir.

Objective: The aim of this study was to examine serotonergic system activity in patients with panic disorder and generalized anxiety disorder by investigating the responses of cortisol, dehydroepiandrosterone-sulfate, and prolactin to escitalopram challenge. Method: Serum concentrations of cortisol, dehydroepiandrosterone-sulfate, and prolactin were measured at baseline and after escitalopram administration in patients with panic disorder (n=24) and generalized anxiety disorder (n=21), and in healthy controls (n=20). Results: Hormonal responses to escitalopram administration in the patients with panic disorder and generalized anxiety disorder were found similar to those of the control group. The baseline dehydroepiandrosterone-sulfate levels were lower in patients with panic disorder and generalized anxiety disorder than those of the control group. Furthermore, dehydroepiandrosterone-sulfate levels were lower in women than those of men in both patients and control groups. Conclusions: There was no significant hormonal response to escitalopram in the patients and control groups, and there was no difference between the groups. The finding that basal dehydroepiandrosterone-sulfate level was lower in the patients with panic disorder and generalized anxiety disorder suggests that this hormone might play a role in the pathophysiology of these disorders.
