Subklaviyan çalma sendromu, vertebral arterin çıkış yerinin proksimalinde veya subklaviyan arterin
stenozu sonucu kan akımının kontralateral vertebral arter ve baziler arterden ters yöne dönmesi
sonucu oluşur. Çoğunlukla semptomsuz olup, tanı karotis-vertebral Doppler ultrasonografi
(USG) ve DSA ile konur. Bâzen de bizim olgumuzdaki gibi çeşitli arka sistem bulguları ile kendini
gösterir. Darlığın derecesine göre medikal veya cerrahi tedavi uygulanır.
42 yaşındaki kadın hastamızda baş ağrısı, bulantı, kusma, diplopi, vertigo, sol kolda ağrı ve kuvvetsizlik,
sol yüz ve kolda hipoestezi şikâyetleri mevcuttu. Yapılan ultrasonografi ve DSA sonucunda
subklaviyan çalma sendromu tanısını aldı. Semptomlar ciddi derecede darlığı olan sol subklaviyan
artere balon stent uygulanması sonucu düzeldi. Olgumuzun semptomlarının başlangıçta hafif ve
kısa sürmesi nedeniyle gözden kaçabileceğini ve arka sistem bulguları olan olgularda subklaviyan
çalma sendromunun akla getirilmesinin gerekliliğini vurgulamak istedik.

Subclavian steal syndrome is defined as occlusion of either of the subclavian arteries beneath the
origin of the vertebral artery; which causes reversing the direction of blood flow to the contralateral
vertebral artery. It is often asymptomatic; and is diagnosed by carotis-vertebral Doppler ultrasonography
and DSA. Sometimes it appears with various posterior circulation findings as in our
case. Reconstructive surgery or medical therapy is applied as to the narrow part. Our case, a 42 [year]s
old woman had headache, nausea, vomiting, diplopia, vertigo, pain in the left arm and weakness,
and hypoesthesia in left face and arm. She was diagnosed as subclavian steal syndrome by
ultrasonography and DSA.
Her symptoms recovered with balloon stent replacement on the left subclavian artery that was excessively
stenotic. We wanted to emphasize by reporting this case that subclavian steal syndrome
may be overlooked when symptoms are soft and duration is short. The necessity to remember the
probability of presence of subclavian steal syndrome in such cases with posterior circulation findings
is mandatory.
