Dikkatsizlik Yakınmasıyla Başvuran Ergenlerin Aldıkları Tanılar

Amaç: Birincil yakınması dikkat eksikliği olan ergen olguların aldıkları tanıların incelenmesi. Yöntem: Ergen psikiyatrisi kliniğine başvuran 43 olgu görüşmelerle değerlendirilmiş ve DSM-IV ölçütlerine göre tanı konulmuştur. Olgular ayrıca Beck Depresyon Envanteri (BDE) ve Sürekli-Durumsal Kaygı Envanteri (SDKE) ile değerlendirilmiştir. Bulgular: 43 ergen (22 erkek 21 kız, yaş ortalaması 15.9) olgudan onbiri (% 25.6) dikkat eksikliği hiperaktivite bozukluğu (DEHB), onikisi (% 27.9) major depresyon, dördü (% 9.3) anksiyete bozukluğu, üçü (% 7.0) anksiyete bozukluğu ile komorbid major depresyon, beşi (% 11.7) DEHB ile komorbid major depresyon, biri de (% 2.3) mental retardasyon tanısı almıştır. Yedi olgu ise (% 16.3) herhangi bir tanı almamıştır. Major depresyon tanısı alan olguların BDE ve SDKE puanları DEHB olan olgulara ve herhangi bir tanı almayan olgulara göre daha yüksektir. Tartışma: Dikkat sorunları ile başvuran bu ergen örnekleminde DEHB ve major depresyon en sık konulan tanılardır. Olguların hemen hemen beşte birinde dikkat bozukluğuna neden olabilecek birden fazla tanı saptanmıştır. Kız olgularda daha çok depresyon ve anksiyete bozukluğu, erkek olgularda daha çok DEHB tanısı konulmuştur. Ergen psikiyatrisine dikkat sorunları nedeniyle başvuran olguların birden çok tanı için taranması gereklidir.
Psychiatric Diagnosis Among Adolescents with Attention Problems
Objective: To evaluate the psychiatric diagnosis of adolescent cases whose primary complaint was attention problems. Method: 43 adolescents (22 males, 21 females; mean age 15.9) who presented to a university clinic with attention problems were assessed with interviews and diagnosis were made according to DSM-IV criteria. Beck Depression Inventory (BDI) and State-Trait Anxiety Inventory (STAI) were also used. Results: 11 (25.6%) cases were diagnosed as Attention Deficit Hyperactivity Disorder (ADHD), 12 (27.9%) cases were diagnosed as major depression, 4 (9.3%) cases were diagnosed as anxiety disorder, 3 (7.0%) cases were diagnosed as comorbid anxiety disorder and major depression, 5 (11.7%) cases were diagnosed as ADHD comorbid with major depression, and 1 (2.3%) case was mentally retarded. There was no psychopathology in 7 (16.3%) cases. Comorbid and pure depression cases had significantly higher BDI and STAI scores compared to cases with ADHD and without psychopathology. Discussion: ADHD and major depression were the most frequent diagnosis in outpatients with attention problems. Almost one fifth of the cases hah more than one diagnosis which can cause this problem. While most of the girls were diagnosed as having major depression and anxiety disorders, the boys were diagnosed as ADHD. Patients who present to adolescent psychiatry clinics because of attention problems must be screened for multiple diagnosis.
