Depresyon tedavisinde yaygın olarak kullanılan venlafaksinin nadir görülen yan etkilerinden birisi de epileptik nöbetlerdir. Bilindiği kadarıyla venlafaksinin, özellikle uzun salınımlı formunun terapötik dozlarda epileptik nöbete neden olduğuna dair bir olgu henüz bildirilmemiştir. Ayrıca antidepresan kullanan hastalarda nöbet geçirme sıklığı üzerine yapılan çalışmalarda, nöbet geçiren hastaların bir ya da daha fazla predispozan faktöre sahip olduğu belirtilmektedir. Epilepsi öyküsü olanlarda psikotrop ilaçların tedavi dozları dahi nöbetleri tetikleyebilmektedir. Bu çalışmada, önceden epilepsi öyküsü olup majör depresyon tanısı alan 34 yaşındaki bir erkek hastada, venlafaksinin uzun salınımlı formunun terapötik dozlarda dahi grand mal epileptik atakları tetikleyebileceği tartışılmıştır. Sonuç olarak; antidepresan tedavi başlanılması düşünülen hastalar, nöbet geçirme riski açısından ayrıntılı olarak değerlendirilmelidir. Yatkınlığı olan hastalara verilen antidepresan ilaçlar dikkatle seçilmeli ve yakından takip edilmelidir.

Venlafaxine is one of the widely prescribed antidepressants used to treat major depression. One of the rarely seen side effects of venlafaxine is “epileptic seizure”. Up to now, as far as we know, there has been no report about seizures precipitated by therapeutic dosage of the “Extended release (XR)” form of venlafaxine. In the studies regarding the incidence of seizure in patients treated with antidepressant, it has been reported that patients experiencing a seizure had one or more predisposing factors. Therapeutic dosages of the psychotropic drug may precipitate epileptic seizures in patients with a history of epilepsy. In this article we have reported a 34 [year]- old male patient diagnosed with major depression and a history of epilepsy, and highlighted the fact that the epileptic seizures were precipitated by the therapeutic dosages of the venlafaxine XR form in this patient. Finally, we propose that patients being considered for antidepressants should be thoroughly screened with regard to predisposing factors that could precipitate seizures. If the patients who have predisposing factors receive the antidepressants, these drugs should be prescribed very carefully and the situation should be monitoring.
