Posterior kortikal atrofi erken dönemde vizüospasyal sorunlarla başlayan ve ana klinik bulguları Gerstmann sendromu, Balint sendromu ve transkortikal sensoriyel afazi olan nadir bir demans sendromudur. Bu yazıda muayenede sağ yan ihmali, Gerstmann sendromu, Balint sendromunun bazı bileşenleri saptanan; beyin SPECT incelemesinde sol temporo-paryeto-oksipital hipoperfüzyon görüntülenen ambidekster posterior kortikal atrofili bir olgu sunulmuştur. Olgumuzda klinik olarak iki yanlı posterior hemisferik bulgular olmasına rağmen, beyin SPECT incelemesinde yalnızca sol temporo-paryeto-oksipital bölgede hipoperfüzyon saptanmıştır. Hem dominant hem nondominant hemisfere ait bulguların tek bir hemisferde toplanması nedeniyle ve tarayabildiğimiz kadarıyla, literatürdeki tek ambidekster posterior kortikal atrofili olgu olması nedeniyle olgumuz sunulmaya değer bulunmuştur.

Posterior cortical atrophy is a rare, slowly progressive dementia characterized by the development of early visual spatial and visual perceptual deficits, often accompanied by features of Balint’s or Gerstmann’s syndromes and transcortical sensory aphasia. Here we present an ambidextrous case who displayed left temporo-parieto-occipital hypoperfusion in single photon emission computerized tomography manifested by right sided neglect, Gerstmann’s syndrome, some features of Balint’s syndrome. The clinical presentation indicated bilateral posterior hemispheric involvement, but only left sided hypoperfusion in the temporo-parieto-occipital region was observed. Briefly, our patient compiled dominant and non-dominant hemisphere functions in the left hemisphere. As far as we know, this is the first report of an ambidextrous patient with posterior cortical atrophy.
