Multipl Sklerozlu Hastalarda Majör Depresyon ve Yaşam Kalitesi İlişkisi

Amaç: Bu çalışmada majör depresyonun multipl sklerozlu (MS) hastalarda yaşam kalitesi üzerine etkisinin araştırılması amaçlanmıştır.
Yöntem: Çalışma örneklemi ruhsal bozukluklar içerisinden sadece majör depresyonu olan (s=20), herhangi bir ruhsal bozukluğu olmayan (s=20) MS hastaları ile herhangi bir ruhsal bozukluğu olmayan sağlıklı bireylerden (kontrol grubu, s=20) oluşturuldu. Psikiyatrik bozukluklar DSM-IV Eksen I Bozuklukları İçin Yapılandırılmış Klinik Görüşme (SCID-I) ile saptandı. Katılımcıların yaşam kalitesi düzeylerini değerlendirmek için Dünya Sağlık Örgütü Yaşam Kalitesi Ölçeği Kısa Formu (WHOQOL-BREF) uygulandı. MS’in neden olduğu özürlülük derecesini belirlemek için Genişletilmiş Özürlülük Durum Skalası (EDSS) kullanıldı.
Bulgular: Gruplar sosyodemografik özellikler yönünden türdeş bulundu. Herhangi bir ruhsal bozukluğu olmayan MS hastalarında WHOQOL-BREF’in fiziksel sağlık alt puanı, kontrol grubundan anlamlı derecede düşüktü. WHOQOL-BREF’in diğer alt alan puanları yönünden bu iki grup arasında anlamlı fark yoktu. Herhangi bir ruhsal bozukluğu olmayanlara göre, majör depresyonu olan MS hastalarında WHOQOL-BREF’in tüm alt puanları anlamlı derecede düşük bulundu. WHOQOL-BREF puanları ile EDSS puanı, yaş ve hastalık süresi arasında anlamlı bir bağıntı bulunmadı.
Sonuç: Bu çalışma bulguları, MS’li hastaların yaşam kalitesinin belirlenmesinde MS’in kendisine ek olarak eşlik eden majör depresyonun da önemli bir etken olduğunu göstermektedir.
Association Between Major Depression and Quality of Life in Patients with Multiple Sclerosis
Objective: In this study, we aimed to investigate the effects of major depression on quality of life (QoL) in patients with multiple sclerosis (MS).
Method: The study sample was composed of MS outpatients with (n=20) only major depression among the psychiatric disorders, MS outpatients without (n=20) any psychiatric disorder, and healthy subjects (control group, n=20) without any psychiatric disorder. The psychiatric disorders were determined by means of the Structured Clinical Interview for DSM-IV (SCID-I). The World Health Organization QoL Assessment-Brief (WHOQOL-BREF) was performed to assess the participants’ QoL levels. The Expanded Disability Status Scale (EDSS) was used to determine degree of disability due to MS.
Results: The groups were similar with regard to sociodemographic factors. MS patients without any psychiatric disorder had a significantly lower physical health subscale score of WHOQOL-BREF than the control group. There was no significant difference between these two groups in terms of other WHOQOL-BREF subscale scores. Compared to MS patients without any psychiatric disorder, all subscale scores of the WHOQOL-BREF was significantly lower in MS patients with major depression. There was no significant correlation between EDSS score, age and disease duration and WHOQOL-BREF scores.
Conclusion: The results of the present study suggests that comorbid major depression is an important factor in addition to MS itself in determining of QoL in patients with multiple sclerosis.
