Hashimoto ansefalopatisi antitiroid antikorlarla ilişkili, otoimmun etiyolojiye bağlı olduğu düşünülen ve steroid tedavisi ile düzelen bir klinik tablodur. Klinik bulgular inme benzeri epizotlar, nöbet, bilinç bozukluğu gibi akut bir şekilde ya da demans, psikoz gibi tablolarla daha sinsi bir şekilde ortaya çıkabilir. Bu yazıda ilerleyici seyirli unutkanlık yakınmasıyla başvuran, başlangıçta Alzheimer tipi demans düşünülen, daha sonra Hashimoto tiroiditine bağlı kognitif bozukluk olarak izlenen bir olgu sunulmuştur. Unutkanlığın dokuz ay önce başladığını belirten olgunun muayenesinde dikkat, sözel ve görsel bellek, hesaplama bozukluğu saptanmış; olguya Alzheimer tipi demans tanısı konularak asetilkolinesteraz inhibitörü başlanmıştır. İzlemde dikkati çeken apati, tremor, ataksi ve saptanan pozitif antitiroid antikorları ile Hashimoto tiroiditi tanısı konulmuş ve tiroidit tedavisi ile bulguları gerilemiştir. Hashimoto ansefalopatisi ya da benzeri durumların nadir görülmesi ve geriye dönüşümlü tablolar olması nedeniyle olgu sunulmaya değer bulunmuştur.

Hashimoto’s encephalopathy is an autoimmune disorder related to thyroid antibodies and is responsive to steroid therapy. Clinical findings may occur acutely as stroke-like episodes, seizures, confusion or slowly, as dementia or psychosis. We report a patient who was admitted for progressive memorycomplaints which was initially assumed to be dementia of the Alzheimer’s type, but later a cognitive disorder associated with Hashimoto’s thyroiditis was diagnosed. The patient complained of memory problems over the previous nine [month]s, and abnormalities in visual and verbal memory and calculation skills were found in her examination. Acetylcholinesterase inhibitor therapy was started with the diagnosis of Alzheimer type of dementia. At her follow-up, apathy, tremor, ataxia and positive antithyroid antibodies were found and she was diagnosed as having Hashimoto thyroiditis. Her symptoms were resolved following treatment for thyroiditis. This case is worthy to be presented, as Hashimoto’s encephalopathy or cognitive symptoms which may accompany Hashimoto thyroiditis could be rare causes of reversible conditions.
