Çocuk Psikiyatrisi Uzmanlık Eğitiminde Performansa Dayalı Değerlendirme

Amaç: Bu yazıda çocuk psikiyatrisi uzmanlık eğitiminde uygulanan Nesnel Örgün Klinik Sınav (NÖKS)
sürecinin ve sınava dair geri bildirimlerin, ilgili literatürle birlikte tartışılması amaçlanmıştır. Yöntem:
NÖKS hazırlık aşamasında sınav uygulama gereçleri oluşturulmuş, sınav akışı planlamış ve standardizasyon
sağlanmaya çalışılmıştır. 2004-2005 eğitim yılında kliniğimizde bulunan iki farklı kıdemdeki
toplam sekiz uzmanlık öğrencisi (UÖ) için NÖKS uygulaması gerçekleştirilmiştir. UÖ'lerin NÖKS
notu, standardize hasta değerlendirme puanına ve 3 kişilik jüriden aldıkları puanların ortalamalarına
dayanarak hesaplanmıştır. Öğretim üyeleri ve UÖ'lerin sınav sonrasında verdikleri sınavla ilgili
geri bildirimler istatistiksel yöntemlerle karşılaştırılmıştır. Sonuçlar:Sınav sürecine ilişkin geri
bildirimler karşılaştırıldığında UÖ'lerin NÖKS uygulamasını; ?sınav öncesinde sürece ilişkin açıklamaların
yeterliliği?, ?standardize hasta ile görüşme için ayrılan süre' ve 'sınavın uygulama becerilerini
değerlendirme düzeyi? alanlarında öğretim üyelerine göre anlamlı düzeyde yetersiz bulduğu saptanmıştır.
Tartışma: NÖKS'ün mezuniyet sonrası çocuk psikiyatrisi eğitiminde kullanımı konusunda
bir ilk deneyim oluşturan uygulamamız, az sayıda UÖ ile gerçekleştirilmesine karşın, hem bireysel,
hem de kurumsal anlamda eğitim gereksinimlerini saptama olanağı sağlamıştır.
Performance-Based Evaluation In Child Psychiatry Residency Training
Objective: This aim of this paper was to present and discuss the Objective Structured Clinical
Examination (OSCE) procedure applied during the child psychiatry residency training along with the
feedbacks regarding the examination in the light of the relevant literature. Method: At the preparation
phase of OSCE, procedure application instruments were developed, progress of the examination was
planned and standardization was conducted. OSCE was applied to eight child psychiatry residency
trainees (RT) at two different seniorities during 2004-2005 education [year]. Grades obtained from
OSCE were calculated based on the average of scores given by the three different academic staff participating
in the evaluation jury and the standardized patient assessment scores. The feedbacks from
the academic staff and the RT were compared by using statistical methods. Results: The comparison
of the feedbacks from the academic staff and the RT regarding the OSCE procedure was significantly
different. RT rated OSCE as incapable in the areas of 'sufficiency of the instructions prior to the examination',
'adequacy of time for the interview with the standardized patient' and 'the competency of the
exam to evaluate performance skills'. Conclusion: The OSCE procedure presented constituted the initial
experience in child psychiatry residency training. Despite the limited number of RT participated
into the study, OSCE enabled to assess individual and institutional educational needs.
