Amaç: Bu çalışma bipolar affektif bozuklukta, mani ve remisyon dönemlerinde oluşan beyin kan akımı değişikliklerinin duruma özgü olup olmadığını kontrollerdeki kan akımı ile karşılaştırarak belirlemek, durumdan bağımsız olarak bipolar affektif bozuklukta yerleşik olabilecek değişiklikleri saptamayı amaçlamaktadır. Yöntem: Çalışmada mani (s=17), remisyon (s= 16) ve kontrol (s=17) olmak üzere toplam 50 denek üzerinde çalışılmıştır. Gruplar arasında yaş ve cinsiyet açısından fark yoktur. Bipolar affektif bozukluk tanısı DSM-IV Eksen I Bozuklukları için Yapılandırılmış Klinik Görüşme ile koyulmuş ve yine bu görüşme ile mani veremisyon grupları birbirinden ayrılmıştır. Kontrol grubu ise normal sağlıklı bireylerden oluşmuştur. Tüm deneklere hangi grupta olduklarını bilmeyen araştırmacılar tarafından 99mTc-HMPAO SPECT uygulanmıştır. Üç grubun kan akımları tüm beyne, serebelluma ve karşı eşdeğer beyin bölgesine oranlanmış değerler olarak karşılaştırılmıştır. Bulgular: Çalışma sonucunda mani ve remisyon grubundaki beyin kan akımı arasında, hiçbir ölçütte anlamlı fark saptanmazken, kontrol grubu ve diğer iki grup arasında anterior-posterior gradientte farklılıklar, sağ temporal lobda belirginleşen perfüzyon değişiklikleri saptanmıştır. Beyin kan akımları ile mevsimsel özellikler, ailesel yüklülük, ilaç kullanımı, hastalık başlangıç yaşı ve hastalık süresi ile ilgili bağıntılar da incelenmiştir. Sonuç: Bu bulguların net bilgiler olarak kliniğe uyarlanabilmesi için daha fazla denek sayısı ile kontrollü çalışmalar yapılmasına ihtiyaç vardır.

Objective: This study was conducted to determine whether the alteration patterns in regional cerebral blood flow in bipolar disorder, if present, are state dependent and to specify the trait characteristics , distinguishing patients from controls in terms of cerebral blood flow. Method: The study comprises 50 subjects of which 17 were bipolar manic, 16 bipolar patients in full remission (with a total of 33 bipolar I patients), and 17 healthy volunteers. The groups were similar in terms of age and gender. DSM-IV Structured Clinical Interview for Diagnosis of Axis I Disorders was performed to verify the diagnosis of bipolar affective disorder. The control group consisted of healthy volunteers. 99m Tc-HMPAO SPECT was performed for all of the subjects. The regional cerebral blood flows relative to the cerebellum and to the total brain blood flow were maesured. Results: There was no statistically significant difference regarding the brain blood flows in any of the measures between mania and remission groups. The control group was statistically different from the bipolar I group, the most prominent alterations being in the right temporal lobe and the anterior-posterior gradient. Conclusion: In order to apply these data for clinical use, further controlled studies with larger patient groups are required.
