Amaç: Unipolar depresyon ve bipolar depresyon gruplarını sosyodemografik ve klinik özellikler açısından
karşılaştırarak birbirlerinden ayırt etmeye yarayacak yordayıcılar saptamaktır.
Yöntem: Araştırmaya 30 major depresif bozukluk, yineleyici (UD) ve 20 bipolar bozukluk major depresif
epizod (BD) tanısı konan hasta alınmıştır. Hastalık şiddeti için Klinik Global İzlenim (KGİ) ölçeği ve depresif
belirtilerin belirlenmesi amacıyla Beck Depresyon Ölçeği (BDÖ) ve Hamilton Depresyon Değerlendirme
Ölçeği- Mevsimsel Duygudurum Bozukluğu İçin Yapılandırılmış Görüşme Kılavuzu (SIGH-SAD) kullanılmıştır.
Bulgular: Bipolar depresyon grubunda daha fazla sayıda epizod, daha fazla hastaneye yatma, ailede
daha fazla bipolar bozukluk öyküsü; UD grubunda daha uzun süreli depresif epizod, daha yüksek oranda
anksiyete bozukluğu komorbiditesi, saptanmıştır. Klinik özellikler açısından depresyon alt tipleri ve intihar
öyküsü farklılığa neden olmamıştır. BDÖ ve SIGH-SAD alt ölçek ve toplam puanları açısından istatistiksel
farklılık saptanmazken, KGİ ile bipolar depresyon grubunda daha şiddetli depresyon gözlenmiştir. UD
grubunda epizod aralarının daha uzun olması ve üzüntü duygusunun, BD grubunda epizod sayısının daha
fazla olması, mevsimsellik, ailede daha fazla bipolar bozukluk öyküsü ve kendini toplumdan soyutlamanın
ayırt edici olduğu saptanmıştır.
Tartışma: Her ne kadar bipolar depresyon ile unipolar depresyonu ayırt etmek zor olsa da, epizod sayısı,
ailede bipolar bozukluk öyküsü, mevsimsellik, duygudurumun niteliği yardımcı olabilmektedir.

Objective: To investigate the discriminating sociodemographic and clinical characteristics of unipolar depresyon and bipolar depresyon.
Method: Our study population consisted of 30 unipolar depresyon patients and 20 bipolar depresyon patients. Patients were diagnosed
with the Structured Clinical Interview for DSM-IV (SCID-CV), and the symptomatology was assessed with Beck Depresyon Inventory (BDI) and
Structured Clinical Interview Guide for the Hamilton Depresyon Rating Scale-Seasonal Affective Disorder Version (SIGH-SAD), and the severity
was determined with Clinical Global Impression (CGI).
Results: Patients in the BD group had more bipolar disorder in the family history, more hospitalization rate, and higher number of previous episodes,
while the duration of the depressive episode was longer; the comorbidity of anxiety disorders was higher in the UD group. Regarding
the clinical characteristics of depresyon, the subtypes and suicidality did not cause any statistical difference between the two groups. Depresyon
was more severe in the BD group than in the UD group according to CGI, but the mean scores of BDI and SIGH-SAD were not different in
the two groups. In discriminating analysis, longer intervals between the episodes and feeling of sadness were discriminating for the UD group;
and higher number of episodes, seasonality, more bipolar disorder in the family and social withdrawal were discriminating for the BD group.
Conclusion: Even though it is difficult to discriminate bipolar depresyon from unipolar depresyon regarding demographical and clinical features,
numbers of episodes, family history for bipolar disorder, seasonality, and nature of the mood seem to be helpful in discriminating these
two disorders.
